维生素D、淋巴细胞亚群与2型糖尿病患者颈动脉斑块的相关性研究

摘　　要：目的:探究2型糖尿病(T2DM)患者并发颈动脉斑块与维生素D水平以及T细胞亚群水平的相关性。方法:收集并测定41例住院的T2DM患者一般临床和生化指标、总25羟维生素D[25(OH)D]水平以及淋巴细胞亚群水平,采用颈动脉彩超测量有无颈动脉斑块形成,并根据有无斑块分为斑块组(24例)和对照组(17例)。结果:斑块组患者年龄大于对照组(P<0.05),总25(OH)D水平和CD3^(+)CD4^(+)T细胞比例均低于对照组(P<0.05)。多因素Logistic回归分析显示,总25(OH)D水平(OR=1.176,95%CI:1.011~1.368)及CD3^(+)CD4^(+)T细胞比例低(OR=1.129,95%CI:1.007~1.266)是颈动脉斑块形成的独立危险因素(P<0.05)。结论:总25(OH)D水平低是T2DM患者颈动脉斑块形成的独立危险因素,可作为颈动脉斑块的预测指标。Objective:To investigate correlation of serum vitamin D level and lymphocyte subsets with presence of carotid plaques in patients with type 2 diabetes(T2DM).Methods:The general clinical and biochemical indexes,total 25(OH)D level and lymphocyte subsets were determined and collected in 41 inpatients with diabetes treated in our hospital.Carotid plaques were detected by carotid color Doppler ultrasound,and then the patients were divided into control group(n=17)and plaque group(n=24)by the presence of plaque detected.Results:Patients with plaques were older than those in the control group(P<0.05),and had lower total 25(OH)D levels and proportions of CD3^(+)CD4^(+)T cells(P<0.05).Multivariate logistic regression analysis showed that low total 25(OH)D level(OR=1.176,95%CI:1.011-1.368)and low proportion of CD3^(+)CD4^(+)T cells(OR=1.129,95%CI:1.007-1.266)were independent risk factors for carotid plaque development(P<0.05).Conclusion:Total 25(OH)D levels are a negatively associated independent risk factor for carotid plaque formation in type 2 diabetes,and may serve as a predictor of carotid plaque.
