芪苈强心胶囊致心率减慢1例  

A case of heart rate slowing induced by Qili Qiangxin Capsules

摘　　要：1例86岁男性患者因退行性心脏瓣膜病住院治疗,因心功能Ⅳ级,给予芪苈强心胶囊1.2 g,口服,3次/天,改善心功能。患者服药4 d后(服用10次,共40粒)心率出现波动,最低41次/分;最高57次/分。患者继续服用两次(共计48粒后),12 h内心率维持在40次/分左右,心率显著降低,同时感乏力,轻度头晕、食欲差。立即停用芪苈强心胶囊,给予心宝丸120 mg,po,tid;左西孟旦注射液12.5 mg+5%葡萄糖注射液250 mL,微量泵泵入st对症治疗。同时查地高辛血药浓度示:地高辛>5.00 ng·mL^(-1)(正常值范围:0~2.0)。15 h后患者心率为70次/分左右。A case of 86 years old male patients with degenerative heart valvular disease in hospital treatment,due to cardiac function class IV,given Qili Qiangxin Capsules 1.2 g,oral,3 times/day,improve cardiac function.After 4 days of taking the medicine(10 times,a total of 40 pills),the patient′s heart rate fluctuated with the lowest rate of 41 beats/min.Maximum 57 times/point.The patient continued to take two doses(48 pills in total),and the heart rate maintained at about 40 beats/min for 12 hours.The heart rate decreased significantly,and the patient felt fatigue weak,mild dizziness and poor appetite.Immediately discontinue Qili Qiangxin Capsules and administer Xinbao Pills 120 mg,po,tid;Levosimendan Injection 12.5 mg+5%Glucose Injection 250 mL,micro pump st Symptomatic treatment.At the same time,digoxin blood concentration showed digoxin>5.00 ng·mL^(-1)(normal value range:0~2.0).After 15 hours,the patient′s heart rate was around 70 beats per minute.It returned to normal.
