宋若会主任诊治急乳蛾特色探析

摘　　要：急乳蛾是耳鼻喉科常见病,现代医学多以抗感染治疗,中医学大多采用清热祛实为治疗大法。宋主任认为急乳蛾系外风引动内热,致“痰涎气血,结聚于咽喉”,其病理转归为阴液耗伤,气凝血涩,风痰内生。在治疗上,郑氏喉科提倡以专病专方之“紫正地黄汤”加减拟方,并注重根据患者的个性化特征层次调治、随宜而施,从而达到养阴清热、疏风散邪、调气活血、逐痰消肿的功效,体现了郑氏喉科未病先防、防病传变的治疗理念,供临床借鉴与探讨。Acute purulent tonsillitis is a common disease in otolaryngology.Anti-infection is the main treatment in modern medicine,while clearing heat and removing excess is the main treatment in traditional Chinese medicine.Director Song believes that the acute purulent tonsillitis is caused by external wind triggering internal heat,leading to“phlegm,saliva,qi and blood accumulating in the throat”.The pathological changes are the consumption of yin fluid,coagulated qi and unsmooth blood circulation,and endogenous wind and phlegm.In treatment,the school of Zheng′s laryngology advocates that the modified prescription for the disease should be based on Zizheng Dihuang Decoction,which is a special prescription for acute purulent tonsillitis,and focuses on adjusting the treatment according to the individual characteristics of patients,so as to achieve the effects of nourishing yin and clearing heat,expelling wind and dispelling evil,regulating qi and activating blood circulation,eliminating phlegm and relieving swelling.It embodies the treatment concept of treating before disease onset and preventing disease transmission from Zheng′s laryngology,which provides reference for clinical practice.
