摘　　要：目的 分析前列腺液中巨噬细胞炎性蛋白-1α(MIP-1α)、单核细胞趋化蛋白-1(MCP-1)表达与慢性前列腺炎的关系。方法 回顾性分析2019年10月至2021年10月在唐山市开滦总医院进行诊治的150例慢性前列腺炎患者的临床资料,将其作为观察组,根据美国国立卫生研究院制定的病理分型方法将其分为Ⅱ型组(n=50)、ⅢA组(n=54)、ⅢB组(n=46),另根据慢性前列腺炎症状指数将其分为轻度组(n=48)、中度组(n=62)、重度组(n=40),并选取同期50例体检合格的健康男性作为对照组。比较不同病理分型、不同症状严重程度的慢性前列腺炎患者与健康对照组人群前列腺液中MIP-1α、MCP-1水平差异,分析前列腺液中MIP-1α、MCP-1表达与病理分型及病情进展的关系。结果 Ⅱ型、ⅢA型、ⅢB型组前列腺液中MIP-1α、MCP-1水平均高于对照组,且Ⅱ型、ⅢA型组高于ⅢB型组,其中Ⅱ型组又高于ⅢA型组(P<0.05)。轻、中、重度组前列腺液中MIP-1α、MCP-1水平均高于对照组,且中、重度组均高于轻度组,其中重度组又高于中度组(P<0.05)。经ROC曲线分析,MIP-1α、MCP-1联合检测应用于慢性前列腺炎的诊断中具有极高的价值。结论 慢性前列腺炎患者的前列腺液中MIP-1α、MCP-1表达水平增高,有助于疾病的早期诊断,在病理分型的鉴别和病情严重程度的评估中具有较高的临床价值。Objective To analyze the relationship between the expression of macrophage inflammatory protein-1α(MIP-1α)and monocyte chemoattractant protein-1(MCP-1)in prostatic fluid and chronic prostatitis.Methods The clinical data of 150 patients with chronic prostatitis who were diagnosed and treated in Kailuan General Hospital of Tangshan City from October,2019 to October,2021 were retrospectively analyzed,and these patients were included as the observation group.According to the pathological classification method developed by the National Institutes of Health,they were further divided into type II group(n=50),IIIA group(n=54),and IIIB group(n=46).According to the chronic prostatitis symptom index,they were further divided into mild group(n=48),moderate group(n=62)and severe group(n=40).50 healthy men who passed the physical examination during the same period were selected as the control group.The differences in levels of MIP-1αand MCP-1 in prostatic fluid between chronic prostatitis patients with different pathological types and severity of symptoms and healthy control groups were evaluated,as well as their relationships to disease progression.Results Levels of MIP-1αand MCP-1 in the prostatic fluid of type II,IIIA and IIIB groups were higher than those of the control group,and levels of type II and IIIA were higher than those of type IIIB group,and levels of type II group were higher than those of type IIIA group(P<0.05).Levels of MIP-1αand MCP-1 in the prostatic fluid of the mild,moderate and severe groups were higher than those of the control group,and the moderate and severe groups were higher than the mild group,and the severe group was higher than the moderate group(P<0.05).According to the ROC curve analysis,the combined detection of MIP-1αand MCP-1 had an extremely high value in the diagnosis of chronic prostatitis.Conclusion The increased expression of MIP-1αand MCP-1 in the prostatic fluid of patients with chronic prostatitis is useful for the early diagnosis of the disease,and has a high clinical value
