高频超声在诊断部队男性官兵精索静脉曲张中的临床应用

摘　　要：目的:探讨部队男性官兵精索静脉曲张(VC)发病情况及高频超声在部队男性官兵精索静脉曲张中的应用价值。方法:选取2021年12月—2022年5月之间在中国人民解放军联勤保障部队第983医院门诊接受高频彩超检查者493例,其中268例基层部队男性官兵作为研究组,225例地方男性作为对照组,应用高频超声检测并记录其平静呼吸时精索静脉曲张最大内径值(DR)、站立位时瓦氏动作试验精索静脉曲张最大内径值(DV)和反流时间(TR)等。后分析研究组病例精索静脉曲张发病情况及入伍时间关系,以及入伍时间与DR、DV及TR关系。结果:研究组精索静脉曲张患病率约42.5%(114/268),明显高于对照组的31.6%(71/225);114例部队官兵VC患者中,VC好发于左侧(70/114,61.4%),Ⅰ级患者最多(48/114,42.1%),114例患者中48例接受手术治疗,超声诊断同手术结果符合率达100.0%(48/48);随着入伍时间的增加精索静脉曲张的患病率也增高,且入伍时间越长VC患者的DR、DV及TR值也随着增高。结论:高频超声对部队男性官兵精索静脉曲张有重要诊断价值,可以明确VC分级,为临床提供可靠的影像依据。Objective To explore the incidence of varicocele(VC)in male officers and soldiers and the application value and analysis of high frequency ultrasound in varicocele in male officers and soldiers.Methods A total of 493 patients who received high frequency color ultrasound examination in the outpatient clinic of the 983 Hospital of the Joint Logistics support Force of the Chinese People's Liberation Army from December 2021 to May 2022 were selected.Among them,268 male officers and soldiers at the grass-roots level served as the study group,and 225 local men served as the control group.The maximum internal diameter of varicocele(DR)during quiet breathing,the maximum internal diameter(DV)and reflux time(TR)of varicocele in Valsalva test were detected and recorded by high frequency ultrasound.T-test was used to compare the differences between the two groups,and univariate analysis of variance was used to evaluate the differences between the two groups.Later,the relationship between the incidence of varicocele and enlistment time,as well as the relationship between enlistment time and DR,DV or TR were analyzed.Results The prevalence rate of varicocele in the study group was about 42.5%(114/268),which was significantly higher than that in the control group(31.6%).Among the 114 cases of VC patients in the army,VC mainly occurred on the left side(70/114,61.4%),grade I patients were the most(48/114,42.1%),and 48 cases of them received surgical treatment.The coincidence rate of ultrasonic diagnosis with surgical results was 100%(48/48).The prevalence of varicocele increased with the increase of enlistment time,and the DR,DV and tr values of VC patients increased with the increase of enlistment time.Conclusion High-frequency ultrasound plays an important role in the diagnosis of varicocele in male officers and soldiers.It can determine the VC grade and provide reliable imaging basis for clinical practice.
