臂踝脉搏波传导速度与左心室舒张功能不全的相关性研究现状

摘　　要：原发性高血压是心脑血管疾病的主要危险因素,但其控制率仍偏低,长期血压控制不良会造成多器官功能不同程度的损害,其中最先受累的靶器官是心脏,对于高血压患者来说,左心室舒张功能受损是心血管不良事件风险增加的预测因素,故早期对舒张功能进行评价是至关重要的,高血压可导致主动脉硬化,长期高血压增加动脉血流动力学负荷,可使动脉顺应性降低,最终导致动脉管壁硬化,此外,动脉僵硬可能与高血压、糖尿病和慢性肾脏疾病的发展形成恶性循环。同时有研究发现左心室舒张功能不全与主动脉硬化之间是相耦联的关系,臂踝脉搏波传导速度是测量动脉硬化的重要指标,现就臂踝脉搏波传导速度在左心室舒张功能不全的高血压患者中的应用价值做一综述。Primary hypertension is a major risk factor for cardiovascular and cerebrovascular diseases,but its control rate is still low.Long-term poor control of blood pressure can result in varying degrees of damage to multiple organ functions,with the heart being the first target organ to be affected.In hypertensive patients,impaired left ventricular diastolic function is a predictor of increased risk of cardiovascular events,so it is crucial to evaluate diastolic function at an early stage.Hypertension can lead to aortic sclerosis,and prolonged hypertension increases the arterial hemodynamic load,which can decrease arterial compliance and ultimately lead to hardening of the arterial wall.In addition,arterial stiffness may form a vicious circle with the development of hypertension,diabetes mellitus,and chronic kidney disease.In addition,arterial stiffness may form a vicious circle with the development of hypertension,diabetes mellitus,and chronic kidney disease.Meanwhile,it has been found that the relationship between left ventricular diastolic insufficiency and aortic atherosclerosis is coupled,and the brachial-ankle PWV is an important indicator of arterial atherosclerosis,and we hereby present a review of the value of brachial-ankle PWV in hypertensive patients with left ventricular diastolic insufficiency.
