摘 要:探讨与分析核磁共振(MR)检查在3种不同体位下踝关节周围韧带和肌腱损伤诊断中的价值。方法:选择我院2020年2月—2022年12月进行诊治的踝关节损伤患者65例作为观察组,同期选择在我院进行常规检查(非外伤)患者65例作为对照组。所有入选者都给予踝关节不同体位下MR检查,记录影像学特征,判定周围韧带和肌腱损伤诊断情况。结果:观察组的美国足和踝关节协会(AOFAS)评分为(79.33±3.22)分,对照组的AOFAS评分为(95.33±2.91)分,观察组的AOFAS评分明显低于对照组(t=14.757,P=0.000);观察组在正常体位、完全跖屈体位、完全背屈体位下的腓骨、距骨骨隧道扩大率都明显高于对照组(P<0.05);观察组在正常体位、完全跖屈体位、完全背屈体位下的踝关节T2值都明显高于对照组(P<0.05);在观察组中,踝关节完全背屈体位下的周围韧带和肌腱损伤检出率分别为16.9%、13.8%,踝关节完全跖屈体位下的周围韧带和肌腱损伤检出率分别为18.5%、12.3%,踝关节正常体位下的周围韧带和肌腱损伤检出率分别为15.4%、13.8%,不同体位下周围韧带和肌腱损伤的检出率对比无明显差异(P>0.05)。结论:MR能很好显示踝关节损伤患者的踝关节稳定性状况,在踝关节3种体位下都能敏感地反映周围韧带和肌腱损伤情况,具有很好的应用价值。Objective:To explore and analysis the value of magnetic resonance(MR)examination in the diagnosis of ligament and tendon injury around ankle joint under three different postures.Methods:From February 2020 to December 2022,65 cases of patients with ankle injury who were diagnosed and treated in a certain hospital were selected as the case group,and the other 65 cases of healthy volunteers who underwent physical examination in a certain hospital were selected as the control group.All participants were given MR examination of ankle joint under different flexion and extension movements,and the imaging characteristics were recorded,and were to determine the diagnosis of peripheral ligament and tendon injury.Results:The AOFAS score of the American Orthopaedic Foot&Ankle Society(AOFAS)in the case group were 79.33±3.22 points,and the AOFAS score of the control group were 95.33±2.91 points.The AOFAS score of the case group were significantly lower than that of the control group(t=14.757,P=0.000).The expansion rate of fibula and talus bone tunnel in the case group were significantly higher than that in the control group(P<0.05)at the normal position,complete plantar flexion position,complete dorsiflexion position.The T2 value of ankle joint in the case group were significantly higher than that in the control group(P<0.05)at the normal position,complete plantar flexion position,complete dorsiflexion position.In the case group,the detection rate of peripheral ligament and tendon injury in the ankle joint fully dorsiflexion position was 16.9%and 13.8%respectively,the detection rate of peripheral ligament and tendon injury in the ankle joint fully plantar flexion position was 18.5%and 12.3%respectively,and the detection rate of peripheral ligament and tendon injury in the ankle joint normal position was 15.4%and 13.8%respectively.There was no significant difference in the detection rate of peripheral ligament and tendon injury in different positions(P>0.05).Conclusion:MR can well display the stability of ankle joint in patie
