经皮针刀筋膜切断术配合中药熏洗方治疗掌腱膜挛缩症的疗效研究

摘　　要：目的探讨B超引导下经皮针刀筋膜切断术配合中药熏洗方治疗掌腱膜挛缩症的疗效。方法选取2019年12月至2022年4月湖州市中医院诊治的60例(109指)掌腱膜挛缩症患者,根据随机数字表法将其分为手术组(30例56指)和联合组(30例53指)。手术组患者给予B超引导下经皮针刀筋膜切断术治疗,联合组患者在手术组治疗基础上联合中药熏洗。比较两组患者的疗效、受累关节伸直受限角度、术后并发症发生率、切口愈合时间及治疗满意度。结果联合组患者的治疗优良率显著高于手术组(χ^(2)=6.575,P=0.010);术后1个月,两组患者的掌指关节、近端指间关节伸直受限角度均明显缩小(P<0.05),且联合组患者的掌指关节、近端指间关节伸直受限角度均显著小于手术组(P<0.05);联合组患者的切口愈合时间显著短于手术组(P<0.05),治疗满意度评分明显高于手术组(P<0.05)。两组患者的并发症发生率比较差异无统计学意义(χ^(2)=0.517,P=0.472)。结论B超引导下经皮针刀筋膜切断术配合中药熏洗方治疗掌腱膜挛缩症患者疗效较好,可有效缩短切口愈合时间,改善患指关节活动度,有助于提高患者的治疗满意度。Objective To study the curative effect of percutaneous needle knife fasciotomy under the guidance of B-ultrasound combined with traditional Chinese medicine fumigation and washing prescription in the treatment of palmar fascia contracture.Methods Sixty patients(109 fingers)with palmar fascia contracture diagnosed and treated in Huzhou Hospital of Traditional Chinese Medicine from December 2019 to April 2022 were selected and divided into operation group(30 cases,56 fingers)and combination group(30 cases,53 fingers)according to random number table method.The operation group was treated with percutaneous needle knife fasciotomy under the guidance of B-ultrasound,and the combined group was treated with traditional Chinese medicine fumigation and washing prescription on the basis of the operation group.The curative effect,restricted angles of extension of the affected joints,postoperative complication rate,incision healing time and treatment satisfaction were compared between the two groups.Results The excellent and good rate of treatment in combination group was significantly higher than that in operation group(χ^(2)=6.575,P=0.010).One month after operation,the restricted angles of extension of metacarpophalangeal joint and proximal interphalangeal joint in both groups were reduced compared with those before operation(P<0.05),and the restricted angles of extension of metacarpophalangeal joint and proximal interphalangeal joint in combined group were significantly smaller than those in operation group(P<0.05).The incision healing time in combined group was significantly shorter than that in operation group(P<0.05),and the treatment satisfaction score was significantly higher than that in operation group(P<0.05).There was no significant difference in the incidence of complications between the two groups(χ^(2)=0.517,P=0.472).Conclusion The percutaneous needle knife fasciotomy under the guidance of B-ultrasound combined with traditional Chinese medicine fumigation and washing prescription for patients with palmar fasc
