摘 要:目的探讨初治肺结核合并空洞患者伴发营养风险的危险因素。方法选取2021年1月至2022年5月于浙江中医药大学附属丽水市中医院初治的164例肺结核伴有空洞患者为研究对象,收集患者的一般临床资料,包括性别、年龄、职业、是否吸烟、是否合并糖尿病、病程、体质量指数、血红蛋白、前白蛋白、白蛋白。根据营养风险筛查2002(nutritional risk screening 2002,NRS 2002)评分将患者分为无营养风险组(NRS 2002评分≤3分)和有营养风险组(NRS 2002评分>3分),采用多因素Logistic回归分析探讨初治肺结核合并空洞患者伴发营养风险的影响因素。结果164例初治肺结核合并空洞患者中有营养风险90例,多因素Logistic回归分析结果显示农民、年龄≥60岁、吸烟、合并糖尿病、病程>1个月均是初治肺结核合并空洞患者伴发营养风险的危险因素(P<0.05)。结论职业为农民、年龄≥60岁、病程>1个月、吸烟、合并糖尿病的初治肺结核合并空洞患者更易发生营养风险。Objective To investigate the risk factors of nutritional risk in newly treated pulmonary tuberculosis patients with cavity.Methods A total of 164 patients with pulmonary tuberculosis with cavity who were newly treated in Lishui Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University from January 2021 to May 2022 were selected as the study objects.General clinical data were collected,including gender,age,occupation,smoking status,diabetes mellitus,disease duration,body mass index,hemoglobin,prealbumin and albumin.Nutritional risk screening 2002(NRS 2002)score was used to classify patients into non-nutritional risk group(NRS 2002 score≤3)and nutritional risk group(NRS 2002 score>3).Multivariate Logistic regression analysis was used to explore the influencing factors of nutritional risk in newly treated pulmonary tuberculosis patients with cavity.Results Among 164 newly treated pulmonary tuberculosis patients with cavity,90 cases had nutritional risk.Multivariate Logistic regression analysis showed that farmers,age≥60 years old,smoking,combined with diabetes mellitus,and disease duration>1 month were all risk factors for nutritional risk in newly treated pulmonary tuberculosis patients with cavity(P<0.05).Conclusion The newly treated pulmonary tuberculosis patients with cavity whose occupation is farmers,age≥60 years old,disease duration>1 month,smoking and diabetes mellitus are more likely to have nutritional risks.
