头颈癌放疗患者吞咽康复管理的最佳证据

摘　　要：目的检索并总结头颈癌放疗患者吞咽困难管理相关证据,为临床开展头颈癌放疗患者吞咽困难管理提供依据。方法系统检索国内外相关指南网站、循证数据库、原始文献数据库、专业协会网站中有关头颈癌放疗患者吞咽困难管理的相关文献,检索时限为建库至2023年1月。由研究者独立对文献质量进行评价,提取并汇总相关证据。结果共纳入文献16篇,其中临床决策3篇、证据总结1篇、指南4篇、系统评价5篇、专家共识3篇,从组建多学科团队、筛查与评估、风险因素、口腔管理、疼痛护理、康复与治疗、饮食指导、随访,8个方面总结22条最佳证据。结论头颈癌放疗患者吞咽困难管理的最佳证据总结形成过程严谨、科学,为临床开展头颈癌放疗患者吞咽困难管理实践提供依据。Objective To search and summarize the evidence for the management of dysphagia in radiotherapy-treated patients with head and neck cancer,thus providing references for the clinical management.Methods Relevant literatures on the management of dysphagia in radiotherapy-treated patients with head and neck cancer were systematically searched from domestic and foreign guidelines and websites,evidence-based databases,original literature databases,and professional association websites from the establishment of the library to January 2023.The researchers independently evaluated the literature quality,and extracted relevant evidences and summarized them.Results A total of 16 articles were included,involving 3 clinical decisions,1 evidence summary,4 guidelines,5 systematic reviews,and 3 expert consensuses.A total of 22 optimal evidences were summarized from 8 aspects,including the establishment of multidisciplinary teams,screening and evaluation,risk factors,oral management,pain care,rehabilitation and treatment,dietary guidance,and follow-up.Conclusion The optimal evidence summary for the management of dysphagia in radiotherapy-treated patients with head and neck cancer is rigorous and scientific,which provides a basis for the clinical management.
