加味六君子汤治疗鼻内镜术后黏膜水肿临床观察

摘　　要：目的探究加味六君子汤在减轻鼻内镜术后黏膜水肿中的作用。方法随机抽取2019年1月—2021年1月于鄱阳县人民医院行鼻窦炎鼻内镜手术治疗的80例患者纳入研究,使用红蓝球分为对照组及观察组,各40例,对照组给予常规西药干预,观察组在此基础上实施加味六君子汤治疗,比较2组患者治疗前后鼻腔黏膜超微结构变化、主观症状评分及黏膜水肿改善效果。结果治疗后,2组环状细胞(GC)、黏膜下腺体开口(GO)水平均较治疗前存在不同程度降低,且观察组低于对照组(P<0.05);2组黏膜纤毛覆盖率较治疗前有不同程度的提升,且观察组高于对照组(P<0.05);2组患者头痛、鼻涕、鼻塞、嗅觉失灵等症状评分较治疗前均下降,且观察组低于对照组(P<0.05);观察组黏膜水肿改善总有效率为97.50%(39/40),明显高于对照组的80.00%(32/40)(P<0.05)。结论鼻内镜术后黏膜水肿治疗过程中采用加味六君子汤进行干预可明显降低疾病主观症状评分,在调节黏膜超微结构的基础上,减轻黏膜水肿效果,临床应用价值较高。Objective To explore the effect of modified Liujunzi decoction in reducing mucosal edema after nasal endoscopic surgery.Methods A random sample of 80 patients with sinusitis and nasal endoscopic surgery treated in Poyang County People’s Hospital from January 2019 to January 2021 were included in the study.They were divided into the control group and the observation group by red and blue balls,with 40 cases in each group.The control group was treated with conventional Western medicine.The observation group was treated with modified Liujunzi decoction on this basis.The changes of nasal mucosal ultrastructure,subjective symptom scores and the improvement effect of mucosal edema between the two groups was compared before and after treatment.Results After treatment,the levels of annular cell(GC)and submucosal gland opening(GO)in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).The mucosal cilia coverage in the two groups were higher than those before treatment,and those in the observation group were higher than those in the control group(P<0.05).The symptom scores of headache,nasal discharge,nasal obstruction and failure of smell in two groups were lower than those before treatment,and the sores of observation group were lower than those of the control group(P<0.05).The total effective rate of mucosal edema improvement in the observation group(97.50%,39/40)was more significantly higher than that in the control group(80.00%,32/40)(P<0.05).Conclusion The intervention of modified Liujunzi decoction during the treatment of mucosal edema after nasal endoscopic surgery can significantly reduce the subjective symptom scores of the disease.On the basis of regulating the ultrastructure of mucosa,it can reduce the effect of mucosal edema and has high clinical application value.
