摘　　要：目的比较腹腔镜经腹膜前法使用免钉合3D补片与Rutkow术治疗老年腹股沟疝患者的效果。方法选取2018年4月至2021年4月我院收治的200例老年腹股沟疝患者进行研究,根据手术方式不同将其分为对照组和观察组,各100例。对照组采用Rutkow术治疗,观察组采用腹腔镜经腹膜前法使用免钉合3D补片治疗。比较两组的治疗效果。结果术后1 d,观察组的丙二醛(MDA)、C反应蛋白(CRP)水平均低于对照组,超氧化物歧化酶(SOD)水平高于对照组(P<0.05)。术后1周及1、3个月,观察组的视觉模拟评分法(VAS)评分低于对照组(P<0.05)。术后1 d,观察组的基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)水平低于对照组(P<0.05)。结论相较于Rutkow术,腹腔镜经腹膜前法使用免钉合3D补片治疗老年腹股沟疝的效果显著,能进一步减轻机体应激反应和疼痛程度,促进MMP-2、MMP-9水平有效降低,值得推广。Objective To compare the effects of laparoscopic preperitoneal method with nail-free 3D patch and Rutkow surgery in the treatment of elderly patients with inguinal hernia.Methods A total of 200 elderly patients with inguinal hernia admitted in our hospital from April 2018 to April 2021 were selected for the study.According to different surgical methods,the patients were divided into control group and observation group,with 100 cases in each group.The control group was treated by Rutkow surgery,and the observation group was treated by laparoscopic preperitoneal method with nail-free 3D patch.The therapeutic effects of the two groups were compared.Results At 1 d after surgery,the levels of malondialdehyde(MDA)and C-reactive protein(CRP)in the observation group were lower than those in the control group,while the level of superoxide dismutase(SOD)was higher than that in the control group(P<0.05).At 1 week and 1 and 3 months after surgery,the Visual Analogue Scale(VAS)score of the observation group was lower than that of the control group(P<0.05).At 1 d after surgery,the levels of matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase-9(MMP-9)in the observation group were lower than those in the control group(P<0.05).Conclusion Compared with Rutkow surgery,laparoscopic preperitoneal method with nail-free 3D patch in the treatment of elderly patients with inguinal hernia has a significant effect,it can further reduce the body's stress response and pain degree,and promote the effective reduction of MMP-2 and MMP-9 levels,which is worth promoting.
