摘  要:[目的]研究清热化痰通腑方对脑梗死合并细菌性肺炎大鼠血清白介素-1β(IL-1β)、内皮素1(ET-1)以及肺组织IL-1β的影响,探讨清热化痰通腑方对脑梗死合并细菌性肺炎大鼠抗炎的作用机制。[方法]将18只大鼠随机分成假手术组、模型组、清热化痰通腑方组,每组6只。采用改良线栓法结合气管注射法建立脑梗死合并细菌性肺炎的大鼠模型,清热化痰通腑方组灌肠给予清热化痰通腑方药液(剂量为7.6 ml/d),假手术组、模型组灌肠给予等量生理盐水,连续给药7 d。第1 d及第7 d测定各组神经功能评分,第7 d采集各组血液标本和脑、肺组织。用HE染色法观察各组脑、肺组织病理变化;采用酶联免疫吸附法(ELISA)检测血清IL-1β、ET-1以及肺组织IL-1β的含量。[结果]清热化痰通腑方组神经功能评分明显降低,与模型组比较差异有显著性意义(P<0.01);清热化痰通腑方能够改善脑梗死合并细菌性肺炎大鼠脑、肺组织病理学损伤及炎症;与模型组比较,清热化痰通腑方组血清IL-1β、ET-1和肺组织IL-1β浓度明显降低,差异有显著性意义(P<0.01)。[结论]清热化痰通腑方可以改善脑梗死合并细菌性肺炎大鼠肺部炎症反应以及神经功能缺损症状,可能与清热化痰通腑方能够降低IL-1β、ET-1表达水平有关。[Objective]To study the effects of Qingre Huatan Tongfu decoction on interleukin-1β(IL-1β),endo⁃thelin-1(ET-1)in serum and IL-1βin lung tissue in rats with cerebral infarction complicated with bacterial pneumo⁃nia to explore the anti-inflammatory effects of Qingre Huatan Tongfu decoction on cerebral infarction complicated with bacterial pneumonia in rats.[Methods]Eighteen rats were randomly divided into sham operation group,model group and Qingre Huatan Tongfu decoction group,with 6 rats in each group.The rat model of cerebral infarction complicated with bacterial pneumonia was established by modified suture method combined with tracheal injection.The Qingre Hua⁃tan Tongfu decoction group was given Qingre Huatan Tongfu decoction(7.6 ml/d)by enema,and the sham operation group and the model group were given the same amount of normal saline by enema for 7 days.On the 1st and 7th day,the neurological function scores of each group were measured.On the 7th day,blood samples and brain and lung tissues of each group were collected.The pathological changes of brain and lung tissues in each group were observed by HE staining.The levels of IL-1β,ET-1 in serum and IL-1βin lung tissue were detected by enzyme-linked immunosorbent assay(ELISA).[Results]The neurological function score of the Qingre Huatan Tongfu decoction group reduced signifi⁃cantly lower than that of the model group(P<0.01).Qingre Huatan Tongfu decoction can improve the pathological injury and inflammation of brain and lung tissue in cerebral infarction complicated with bacterial pneumonia in rats.The levels of IL-1β,ET-1 in serum and IL-1βin lung tissue of Qingre Huatan Tongfu decoction group reduced significantly than those of model group(P<0.01).[Conclusion]Qingre Huatan Tongfu decoction can improve the inflammatory reaction and neurological deficit symptoms of cerebral infarction complicated with bacterial pneumonia in rats.It may be related to reduction of the expression levevl of IL-1βand ET-1.
