摘 要:目的探讨多层螺旋CT血管成像(CTA)对椎-基底动脉开窗畸形的诊断价值。方法选取我院1899例行头颈部CTA检查的患者,所有患者均采用本院的256层螺旋CT进行血管成像检查,回顾性分析椎-基底动脉开窗畸形的检出率、发生部位、形态及并发其他血管异常情况。结果1899例头颈部CT血管成像中,检出56例椎-基底动脉开窗畸形(检出率2.9%),其中基底动脉32例(近段15例、中段9例、远段8例)、椎动脉24例(左侧椎动脉15例、右侧椎动脉9例;颅内型5例、颅外型13例、颅内-外型6例);椎-基底动脉开窗畸形按形态可分为裂隙型(14处)、凸透镜型(31处)及重复型(11处);另外,56例椎-基底动脉开窗畸形中,11例伴有动脉瘤。结论多层螺旋CT的血管成像能够准确显示椎-基底动脉开窗畸形的发生部位、形态、毗邻关系,进一步的了解椎-基底动脉的开窗畸形。Objective To investigate the diagnostic value of multi-slice spiral CT angiography(CTA)in vertebrobasilar artery fenestration.Methods 1899 cases of head and neck CTA examination were performed in our hospital.All patients were examined with 256 slice spiral CT angiography.The detection rate,location,shape and other vascular abnormalities of vertebrobasilar artery fenestration were analyzed retrospectively.Results In 1899 cases of head and neck CT angiography,56 cases of vertebrobasilar artery fenestration were detected(the detection rate was 2.9%).There were 32 cases of basilar artery(the proximal,15 cases,middle segment,9 cases,distal segment,8 cases),24 cases of vertebral artery(left vertebral artery,15 cases,right vertebral artery,9 cases;intracranial type,5 cases,extracranial type,13 cases,intracranial-external type,6 cases).According to the shape,the vertebrobasilar artery fenestration deformity can be divided into fissure type(14 places),convex lens type(31 places)and repeated type(11 places).In addition,among 56 cases of vertebrobasilar artery fenestration,11 cases were accompanied by aneurysms.Conclusion Multi slice spiral CT angiography can accurately show the location,shape and adjacent relationship of artery fenestration.Through this retrospective study and analysis,we can further understand the vertebrobasilar artery fenestration deformity,especially different forms of fenestration deformity.
