摘 要:目的探讨维生素D联合低血糖生成指数膳食管理在妊娠期糖尿病患者中的应用效果。方法选取2019年10月至2021年10月收治的90例妊娠期糖尿病患者,以随机数字表法将其分为对照组和观察组,各45例。对照组采用常规饮食干预,观察组采用维生素D联合低血糖生成指数膳食管理。比较两组的临床效果。结果治疗后,观察组的空腹胰岛素(FINS)水平、胰岛素抵抗指数(HOMA-IR)低于对照组,胰岛素分泌指数(HOMA-β)、胰岛素敏感指数(ISI)高于对照组(P<0.05)。治疗后,观察组的丙二醛(MDA)、脂质过氧化物(LPO)水平低于对照组,总抗氧化能力(TAC)、谷胱甘肽(GSH)水平高于对照组(P<0.05)。观察组患者不良结局总发生率低于对照组(P<0.05)。观察组胎儿不良结局总发生率低于对照组(P<0.05)。结论对妊娠期糖尿病患者采用维生素D联合低血糖生成指数膳食管理有助于减轻胰岛素抵抗和氧化应激反应,促进母婴结局改善。Objective To explore the application effect of vitamin D combined with low glycemic index dietary management in patients with gestational diabetes mellitus.Methods A total of 90 patients with gestational diabetes mellitus admitted from October 2019 to October 2021 were selected and divided into control group and observation group by random number table method,with 45 cases in each group.The control group used routine dietary intervention,and the observation group used vitamin D combined with low glycemic index dietary management.The clinical effects of the two groups were compared.Results After treatment,the fasting insulin(FINS)level and insulin resistance index(HOMA-IR)in the observation group were lower than those in the control group,and the insulin secretion index(HOMA-β)and insulin sensitivity index(ISI)were higher than those in the control group(P<0.05).After treatment,the levels of malondialdehyde(MDA)and lipid peroxide(LPO)in the observation group were lower than those in the control group,and the levels of total antioxidant capacity(TAC)and glutathione(GSH)were higher than those in the control group(P<0.05).The total incidence of adverse outcomes in the observation group was lower than that in the control group(P<0.05).The total incidence of fetal adverse outcomes in the observation group was lower than that in the control group(P<0.05).Conclusion The vitamin D combined with low glycemic index dietary management applied in patients with gestational diabetes mellitus can help to reduce insulin resistance and oxidative stress,and promote the improvement of maternal and infant outcomes.
