摘　　要：目的探讨阿司匹林联合地屈孕酮治疗多囊卵巢综合征(PCOS)患者的临床效果及对血清miR-224、miR-206、最大流速(Vmax)的影响。方法选取2019年5月至2020年5月我院收治的300例PCOS患者为研究对象,根据随机数字表法将其分成对照组与观察组,每组150例。对照组采用地屈孕酮治疗,观察组在对照组治疗基础上加用阿司匹林。比较两组的治疗效果。结果治疗后,观察组的miR-224、miR-206、阻力指数(RI)均显著低于对照组,间质动脉Vmax显著高于对照组(P<0.05)。治疗后,观察组的子宫内膜厚度、闭锁卵泡直径、宫颈黏液评分均明显高于对照组(P<0.05);观察组的排卵率、妊娠率均明显高于对照组(P<0.05)。治疗后,观察组的空腹胰岛素(FINS)、餐后2 h胰岛素(2 h INS)、胰岛素抵抗指数(HOMA-IR)均明显低于对照组(P<0.05);观察组的治疗总有效率明显高于对照组(P<0.05)。结论阿司匹林联合地屈孕酮治疗PCOS的效果显著,能有效改善患者血清miR-224、miR-206表达水平与卵巢血流指标,促进子宫内膜发育与卵巢排卵,改善胰岛功能。Objective To explore the clinical effect of aspirin combined with dydrogesterone in the treatment of polycystic ovary syndrome(PCOS)patients and its influences on serum miR-224,miR-206 and maximum velocity(Vmax).Methods A total of 300 PCOS patients admitted in our hospital from May 2019 to May 2020 were selected as the study objects and divided into control group and observation group according to random number table method,with 150 cases in each group.The control group was treated with dydrogesterone,and the observation group was treated with aspirin on the basis of control group.The therapeutic effects of the two groups were compared.Results After treatment,the miR-224,miR-206 and resistance index(RI)in the observation group were significantly lower than those in the control group,and interstitial artery Vmax was significantly higher than that in the control group(P<0.05).After treatment,the endometrial thickness,atretic follicle diameter and cervical mucus score in the observation group were significantly higher than those in the control group(P<0.05);the ovulation rate and pregnancy rate of the observation group were significantly higher than those of the control group(P<0.05).After treatment,the fasting insulin(FINS),2 h postprandial insulin(2 h INS)and homeostatis model assessment of insulin resistance(HOMA-IR)in the observation group were significantly lower than those in the control group(P<0.05);the total effective rate of treatment of the observation group was significantly higher than that of the control group(P<0.05).Conclusion Aspirin combined with dydrogesterone has a significant effect in the treatment of PCOS,which can effectively improve the expression levels of serum miR-224 and miR-206 and ovarian blood flow indicators,promote endometrial development and ovarian ovulation,and improve the islet function.
