摘 要:目的探讨利多卡因吸入式表面麻醉在留置胃管中的作用。方法选取2020年2月至2021年9月行经腹腔手术或涉及肠道手术前留置胃管152例患者为研究对象,按随机数表分为对照组(77例)和利多卡因组(75例)。对照组和试验组利用生理盐水或利多卡因吸入式麻醉鼻咽部后常规留置胃管,比较两组一次性胃管成功率、血压、心率变化及对胃管刺激反应程度情况。结果两组均顺利留置胃管,无利多卡因药物相关不良反应发生。利多卡因组一次置管成功率明显高于对照组,且对生命体征指标影响较小(P<0.05)。对照组收缩压、舒张压变化值明显高于利多卡因组(P<0.05)。对照组插管前后心率变化值明显高于利多卡因组(P<0.05),同时插管过程中不适症状及疼痛程度明显减轻(P<0.05)。结论留置胃管前应用利多卡因吸入式麻醉能有效降低患者血压、心率波动,减轻插管所造成的不适症状,提高留置胃管的成功率。Objective To explore the effect of lidocaine-inhalation anesthesia on indwelling gastric tube.Methods A total of 152 patients who underwent peritoneal surgery or enteric operation with indenture gastric tube before operation from February 2020 to September 2021 were selected as the study objects,and were divided into control group(77 cases)and lidocaine group(75 cases)according to the random number table.Normal saline or lidocaine inhalation anesthesia was used in the control group and the experimental group,and the success rate of one-time gastric tube,blood pressure,heart rate changes and the degree of response to gastric tube stimulation were compared between the two groups.Results Gastric catheterization was successfully placed in both groups,and no lidocaine drug-related adverse reactions occurred.The success rate of one cathedration in lidocaine group was significantly higher than that in control group,and the vital signs were less affected(P<0.05).The changes of systolic and diastolic blood pressure in control group were significantly higher than those in lidocaine group(P<0.05).The heart rate change before and after intubation in the control group was significantly higher than that in the lidocaine group(P<0.05),and the discomfort and pain were significantly reduced during intubation(P<0.05).Conclusion Lidocaine-inhalation anesthesia before gastric tube indwelling can effectively reduce the fluctuations of blood pressure and heart rate,alleviate discomfort caused by intubation,and improve the first-attempt success rate.
