咪喹莫特联合血卟啉光动力治疗光化性角化病的临床效果

摘　　要：目的:探讨光化性角化病患者采用咪喹莫特联合血卟啉光动力治疗的临床效果。方法:按照随机数字表法将2020年10月—2022年10月宜春市人民医院收治的60例光化性角化病患者分为对照组与观察组,各30例。对照组采用咪喹莫特乳膏外涂,观察组采用咪喹莫特联合血卟啉光动力方法治疗。比较两组的临床效果,治疗前后的皮损面积、VISIA皮肤检测,治疗后的复发情况,以及治疗期间不良反应发生情况。结果:观察组患者临床总有效率为93.33%,显著高于对照组的73.33%(P<0.05)。与治疗前比较,治疗后两组患者皮损面积均缩小,且观察组显著小于对照组(P<0.05)。观察组患者复发率为0,显著低于对照组的30.00%(P<0.05)。与治疗前比较,治疗后两组患者VISIA皮肤检测各项指标均升高,且观察组均高于对照组(P<0.05)。两组患者不良反应总发生率比较差异无统计学意义(P>0.05)。结论:咪喹莫特联合血卟啉光动力治疗光化性角化病效果确切,能改善皮损情况,降低患者复发风险,同时不增加不良反应发生。Objective:To investigate the clinical effect of Imiquimod combined with Hematoporphyrin photodynamic therapy in patients with actinic keratosis.Method:According to random number table method,60 patients with actinic keratosis treated in Yichun People's Hospital from October 2020 to October 2022 were divided into control group and observation group,with 30 cases in each group.Patients in the control group were treated with Imiquimod Cream,while patients in the observation group were treated with Imiquimod combined with Hematoporphyrin photodynamic therapy.The clinical effect,lesion area,VISIA skin detection before and after treatment,recurrence after treatment,and the occurrence of adverse reactions during treatment were compared between the two groups.Result:The total clinical efficiency of patients in the observation group was 93.33%,which was significantly higher than 73.33%of the control group(P<0.05).Compared with before treatment,the skin area of both groups decreased after treatment,and the observation group was significantly smaller than that of the control group(P<0.05).The recurrence rate of patients in the observation group was 0,which was significantly lower than 30.00%in the control group(P<0.05).Compared with before treatment,the VISIA skin test indicators of both groups of patients increased after treatment,and the observation group were higher than those of the control group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of Imiquimod and Hematoporphyrin photodynamic therapy has a definite therapeutic effect on actinic keratosis,which can improve the condition of skin lesions,reduce the risk of recurrence in patients,and not increase the occurrence of adverse reactions.
