摘 要:目的:探讨糖尿病患者前列腺切除术后尿路感染的影响因素。方法:采用随机抽样法选择2019年1月—2022年3月石狮市医院收治的100例糖尿病合并前列腺切除术患者进行研究,根据术后感染情况分为感染组(n=22),未感染组(n=78),利用多因素logistic回归分析糖尿病患者前列腺切除术后尿路感染的影响因素及术前血糖对术后尿路感染的预测价值。结果:所有患者均成功完成手术,术后发现22例发现发生尿路感染,转往相关科室进行治疗。单因素分析,两组患者体重指数、年龄、高血压、术前尿培养、前列腺手术史比较,差异均无统计学意义(P>0.05);两组术前血糖、术前尿常规白细胞计数、手术时间比较,差异均有统计学意义(P<0.05);多因素非条件logistic回归分析显示,术前血糖、术前尿常规白细胞计数、手术时间均是糖尿病患者前列腺切除术后发生尿路感染的独立危险因素(P<0.05);ROC曲线显示,术前血糖截断值为6.6 mmol/L时,对糖尿病患者前列腺切除术后发生尿路感染的预测价值最高。结论:糖尿病患者前列腺切除术后发生尿路感染的影响因素涉及多方面,且术前血糖对糖尿病患者前列腺切除术后发生尿路感染有一定预测价值。Objective:To investigate the influencing factors of urinary tract infection in diabetic mellitus patients after prostatectomy.Method:A total of 100 diabetic mellitus patients with prostatectomy admitted to Shishi Hospital from January 2019 to March 2022 were selected by random sampling method for this study.According to postoperative infection,they were divided into infected group(n=22)and uninfected group(n=78).Multivariate logistic regression was used to analyze the influencing factors of urinary tract infection after prostatectomy in diabetic mellitus patients and the predictive value of preoperative blood glucose on postoperative urinary tract infection. Result: All patients successfully completed the operation, and 22 cases were found to have urinary tract infection after operation, and were transferred to relevant departments for treatment. By univariate analysis, there were no significant differences in body mass index, age, hypertension, preoperative urine culture and history of prostate surgery between the two groups (P>0.05);there were significant differences in preoperative blood glucose, preoperative urine routine white blood cell count and operation time between the two groups (P<0.05);multivariate logistic analysis showed that preoperative blood glucose, preoperative routine urine white blood cell count and operative time were independent risk factors for urinary tract infection after prostatectomy in diabetic mellitus patients (P<0.05);the ROC curve showed that the preoperative blood glucose cutoff value of 6.6 mmol/L had the highest predictive value for urinary tract infection after prostatectomy in diabetic patients. Conclusion: There are many factors affecting the occurrence of urinary tract infection in diabetic mellitus patients after prostatectomy, and preoperative blood glucose has a certain predictive value for the occurrence of urinary tract infection in diabetic mellitus patients after prostatectomy.
