摘 要:脓毒性心肌病(septic cardiomyopathy,SCM)是脓毒症休克初期所出现的可逆性心肌损伤,其主要特点为左心室扩大、射血分数降低及存活者心功能7~10 d可恢复正常。其在脓毒血症中的发病率逐年升高,死亡率也相对增加。但其具体发病机制尚不明确,考虑是线粒体损伤、炎症反应、钙离子失衡等多种因素共同作用的结果。目前针对SCM主要以纠正脓毒症休克为基础,以血流动力学和组织灌注为导向的支持性治疗。本文阐述了SCM相关研究的最新发病机制及具体治疗方案,旨在为SCM的诊治提供参考依据。Septic cardiomyopathy(SCM)is a reversible myocardial injury that occurs at the beginning of septic shock.Its main characteristic is left ventricle enlargement,ejection fraction decreased and survivors can be restored to normal cardiac function 7-10 d.Its incidence in sepsis is increasing year by year,and its mortality is also increasing relatively.However,the specific pathogenesis is not clear,and it is considered to be the result of many factors such as mitochondrion damage,inflammation and calcium ion imbalance.At present,SCM is a hemodynamics and tissue perfusion oriented supportive therapy based on the correction of septic shock.This article reviewed the latest pathogenesis and specific treatment of SCM related research,in order to provide reference for the diagnosis and treatment of SCM.
