MUC5AC、TGF-β_(1)、CD69在慢性鼻窦炎鼻息肉组织中的表达及临床意义

摘　　要：目的:分析黏蛋白5AC(mucin 5AC,MUC5AC)、转化生长因子-β_(1)(transforming growth factor-β_(1),TGF-β_(1))、CD69在慢性鼻窦炎鼻息肉组织中的表达及临床意义。方法:选取2018年8月—2021年8月宁德师范学院附属宁德市医院收治的120例慢性鼻窦炎患者,根据患者是否伴有鼻息肉分为不伴鼻息肉组(n=60)、伴鼻息肉组(n=60),另外选取于本院进行健康体检的志愿者为对照组(n=52)。通过实时荧光定量PCR法、酶联免疫吸附法、流式细胞仪检测MUC5AC、TGF-β_(1)、CD69。分析MUC5AC、TGF-β_(1)、CD69表达变化与慢性鼻窦炎鼻息肉的关系及三者的相关性。结果:与对照组相比,不伴鼻息肉组、伴鼻息肉组MUC5AC、TGF-β_(1)、CD69表达水平均更高(P<0.05);与不伴鼻息肉组相比,伴鼻息肉组MUC5AC、TGF-β_(1)、CD69表达水平均更高(P<0.05)。与轻度组相比,中度组、重度组MUC5AC、TGF-β_(1)、CD69表达水平均更高(P<0.05);与中度组相比,重度组MUC5AC、TGF-β_(1)、CD69表达水平均更高(P<0.05)。Pearson相关性分析显示,MUC5AC、TGF-β_(1)之间呈正相关(r=0.524,P=0.001);MUC5AC、CD69之间呈正相关(r=0.529,P=0.001);TGF-β_(1)、CD69之间呈正相关(r=0.554,P=0.001)。ROC曲线显示,三项联合对慢性鼻窦炎鼻息肉的诊断价值高于MUC5AC、TGF-β_(1)、CD69单项诊断(P<0.05)。结论:MUC5AC、TGF-β_(1)、CD69高表达于慢性鼻窦炎鼻息肉患者,随着病情加重,表达水平逐渐升高,且MUC5AC、TGF-β_(1)、CD69之间具有正相关性。Objective:To analyze the expression and clinical significance of mucin 5AC(MUC5AC),transforming growth factor-β_(1)(TGF-β_(1))and CD69 in nasal polyp tissue of chronic sinusitis.Method:A total of 120 patients with chronic sinusitis admitted to Ningde Municipal Hospital of Ningde Normal University from August 2018 to August 2021 were selected and divided into without nasal polyp group(n=60)and with nasal polyp group(n=60)according to whether the patients were accompanied by nasal polyp.In addition,volunteers who underwent health examination in our hospital were selected as the control group(n=52).MUC5AC,TGF-β_(1) and CD69 were detected by real-time fluorescence PCR,enzyme linked immunosorbent assay and flow cytometer.The relationship between the expression of MUC5AC,TGF-β_(1),CD69 and nasal polyp of chronic sinusitis,and the correlation of the three were analyzed.Result:Compared with the control group,the expression levels of MUC5AC,TGF-β_(1) and CD69 were higher in the with nasal polyp group and the without nasal polyp group(P<0.05).The expression levels of MUC5AC,TGF-β_(1) and CD69 in the with nasal polyp group were higher than those in the without nasal polyp group(P<0.05).Compared with mild group,the expression levels of MUC5AC,TGF-β_(1) and CD69 were higher in moderate and severe groups(P<0.05).Compared with moderate group,the expression levels of MUC5AC,TGF-β_(1) and CD69 were higher in severe group(P<0.05).Pearson correlation analysis showed that there were positive correlation between MUC5AC and TGF-β_(1)(r=0.524,P=0.001),MUC5AC and CD69(r=0.529,P=0.001),TGF-β_(1) and CD69(r=0.554,P=0.001).ROC curve showed that the diagnostic value of three combined in nasal polyp of chronic sinusitis was higher than that of MUC5AC,TGF-β_(1) and CD69(P<0.05).Conclusion:MUC5AC,TGF-β_(1) and CD69 are highly expressed in patients with nasal polyp of chronic sinusitis,and the expression levels gradually increase with the worsening of the disease,and there is a positive correlation between MUC5AC,TGF-β_(1) and CD69
