摘  要:目的:探究血液灌流对维持性血液透析患者毒素清除及微炎症的影响。方法:将2019年3月—2020年3月北京市普仁医院收治的82例维持性血液透析患者根据随机数字表法分为对照组(n=41)和观察组(n=41)。对照组常规进行血液透析治疗,观察组在血液透析基础上进行血液灌流治疗。比较两组皮肤瘙痒发生率、治疗前后毒素(小分子及中大分子毒素)水平及微炎症指标[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)及白细胞介素-6(IL-6)]。结果:观察组皮肤瘙痒总发生率显著低于对照组,差异有统计学意义(P<0.05)。治疗前两组毒素水平及微炎症指标比较,差异均无统计学意义(P>0.05);治疗12、24周后,两组毒素水平及微炎症指标均显著降低,观察组均显著低于对照组,差异均有统计学意义(P<0.05)。结论:血液灌流可有效促进维持性血液透析患者的毒素清除及微炎症状态的控制,对皮肤瘙痒有较好的控制效果。Objective:To investigate the effect of hemoperfusion on toxin clearance and microinflammation in maintenance hemodialysis patients.Method:A total of 82 patients with maintenance hemodialysis admitted to Beijing Puren Hospital from March 2019 to March 2020 were divided into control group(n=41)and observation group(n=41)according to random number table method.The control group was routinely treated with hemodialysis,and the observation group was treated with hemoperfusion based on hemodialysis.The incidence of pruritus,before and after treatment of levels of toxin(small molecule and medium giant molecule toxin)and microinflammation indexes[C reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-6(IL-6)]were compared between the two groups.Result:The total incidence of pruritus in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in toxin levels and microinflammation indexes between the two groups(P>0.05).After 12 and 24 weeks of treatment,the toxin levels and microinflammation indexes in both groups were significantly decreased,and those in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Hemoperfusion can effectively promote the clearance of toxin and the control of microinflammation state in maintenance hemodialysis patients,and has a good control effect on skin pruritus.
