摘　　要：目的:探讨微通道经皮肾镜取石术(MPCNL)治疗输尿管结石的效果及其安全性评价。方法:选择2021年3月—2022年9月麻城市人民医院收治的92例输尿管上段结石患者作为研究对象,按照随机数字表法分为MPCNL组(n=46,予以MPCNL治疗)和URL组(n=46,予以经输尿管镜碎石术治疗)。比较两组手术情况、临床疗效及术后并发症发生情况。结果:MPCNL组手术时间明显长于URL组,住院时间短于URL组,差异均有统计学意义(P<0.05);但两组术中估计出血量、术后胃肠功能恢复时间比较,差异均无统计学意义(P>0.05)。MPCNL组一次性碎石成功率高于URL组,但两组比较,差异无统计学意义(P>0.05);MPCNL组术后1个月结石排净率明显高于URL组,差异有统计学意义(P<0.05)。MPCNL组并发症总发生率明显低于URL组(P<0.05)。结论:MPCNL治疗输尿管上段结石效果确切,可改善手术情况,安全性好。Objective:To evaluate the effect and safety of mini-percutaneous nephrolithotomy(MPCNL)in the treatment of ureteral calculus.Method:A total of 92 patients with upper ureteral calculus admitted to People's Hospital of Macheng City from March 2021 to September 2022 were selected as the study objects,and were divided into MPCNL group(n=46,treated with MPCNL)and URL group(n=46,treated with ureteroscopic lithotripsy)according to random number table method.The operation situation,clinical effect and postoperative complications were compared between the two groups.Result:The operation time in MPCNL group was significantly longer than that in URL group and the hospital stay in MPCNL group was shorter than that in URL group,the differences were statistically significant(P<0.05).However,there were no significant differences between the two groups in the intraoperative bleeding volume and postoperative gastrointestinal function recovery time(P>0.05).The success rate of one-time lithotriptic in MPCNL group was higher than that in URL group,but there was no statistical significance between the two groups (P>0.05). The 1-month stone removal rate in MPCNL group was significantly higher than that in URL group, the difference was statistically significant (P<0.05). The total complication rate of MPCNL group was significantly lower than that of URL group (P<0.05). Conclusion: MPCNL is effective in the treatment of upper ureteral calculus, and can improve the operation situation with good safety.
