摘 要:早产儿脑白质损伤是指由围生期缺血、缺氧及炎症反应导致,影响脑发育进程,导致神经系统后遗症,对家庭和社会造成极大负担。颅脑彩超是早产儿初始和连续神经影像学检查的首选方式,具有操作便捷、重复性强等优势,可以评估脑损伤随时间的演变过程,以及大脑的生长和成熟情况。还可以通过动态监测大脑中动脉的血流参数,反映早产儿脑组织灌注,有助于评估脑白质损伤及神经预后。本文就早产儿脑白质损伤的脑血流变化及颅脑彩超在其中的应用进展做一综述。White matter injury in preterm infants is caused by perinatal ischemia,hypoxia and inflammation,which affects the process of brain development and leads to neurological sequelae,causing great burden to the family and society.Craniocerebral color ultrasound is the preferred method for initial and continuous neuroimaging examination of preterm infants.It has the advantages of convenient operation and high repeatability,and can evaluate the evolution of brain injury over time,as well as the growth and maturity of the brain.It can also dynamically monitor the blood flow parameters of middle cerebral artery to reflect the perfusion of brain tissue in preterm infants,which is helpful to evaluate the prognosis of white matter injury and nerve.This article reviews the changes of cerebral blood flow in premature infants with white matter injury and the application of craniocerebral color ultrasound.
