丹蛭降糖胶囊联合羟苯磺酸钙治疗气阴两虚夹瘀型非增殖期糖尿病视网膜病变的临床疗效观察

摘　　要：目的 观察丹蛭降糖胶囊联合羟苯磺酸钙治疗气阴两虚夹瘀型非增殖期糖尿病视网膜病变(NPDR)的临床疗效。方法 检测气阴两虚夹瘀型NPDR患者的血糖血脂、糖基化血红蛋白A1c(HbAlc)、空腹C肽(C-P)、视黄醇结合蛋白(RBP)、同型半胱氨酸(HCY)、血管紧张素转化酶(ACE)、胱抑素C(CysC)水平并做眼底检查。对照组给予基础用药+羟苯磺酸钙胶囊治疗,观察组在此基础上加丹蛭降糖胶囊。对两组治疗前后上述指标及变化进行对比,评价其临床疗效。结果 治疗后,观察组总有效率、眼底积分和中医各证候积分均明显优于对照组(P <0.05或P <0.01);眼底渗出、微血管瘤、出血情况均明显好转。观察组的FPG、2hPG、HbAlc、TG、TC、LDL-C、RBP、HCY、ACE、CysC水平降幅明显高于对照组,C-P水平升高(P <0.05或P <0.01)。结论 丹蛭降糖胶囊联合羟苯磺酸钙疗效确切,可降低患者血糖、血脂、血清Hcy、CysC、ACE、RBP,升高C-P水平,明显改善眼底症状及中医症状。Objective To observe the clinical effect of Danzhi Jiangtang capsule combined with calcium dobesilate in the treatment of non-proliferative diabetic retinopathy(NPDR) with Qi and Yin deficiency and blood stasis.Methods The levels of blood glucose,blood lipid,glycosylated hemoglobin A1c,fasting C-peptide,retinol-binding protein,homocysteine,angiotensin converting enzyme and cystatin C in NPDR with Qi and Yin deficiency and blood stasis were detected.The control group was given basic medication + calcium dobesilate capsules,and the treatment group was added Danzhi Jiangtang capsules on the basis of this.The above indexes and their changes after treatment were compared between the two groups to evaluate the clinical efficacy.Results The total effective rate,fundus score and TCM syndrome score of the treatment group were significantly better than those of the control group(P<0.05,P<0.01).Fundus exudation,microangioma,bleeding were significantly relieved.After the treatment,the levels of FPG,2hPG,HbA1c,TG,TC,LDL-C,RBP,ACE,HCY and Cys C in the treatment group decreased significantly compared with the control group,while the level of C-P was significantly increased(P<0.05 or P<0.01).Conclusion Danzhi Jiangtang capsule combined with calcium dobesilate has definite curative effect,reducing blood glucose,blood lipid,serum Hcy,CysC,ACE,RBP,increasing the level of C-P,and significantly improving fundus symptoms and TCM symptoms.
