摘　　要：目的探讨二妙散加味联合医用臭氧局部灌注对痛风性关节炎(GA)患者疼痛和关节活动度的影响。方法采用随机数表法将88例GA患者分为对照组与研究组,各44例。对照组予秋水仙碱片及双氯酚酸钠肠溶胶囊治疗,研究组予二妙散加味联合医用臭氧局部灌注治疗,均治疗3 d,治疗后观察7 d。比较2组治疗3 d后的疗效,症状缓解时间,治疗前、治疗3 d后的疼痛程度、关节活动评分、血尿酸(BUA)、血沉(ESR)、丙氨酸氨基转移酶(ALT)、黄嘌呤氧化酶(XOD)、超氧化物歧化酶(SOD)及炎症反应指标。结果治疗3 d后,研究组总有效率(93.18%,41/44)高于对照组(72.73%,32/44)(P<0.05);与对照组比较,研究组症状缓解时间更短(P<0.05)。治疗3 d后,2组外周血ESR、血清5-羟色胺(5-HT)、前列腺素E2(PGE2)、BUA、X OD、环氧合酶-2(COX-2)、核苷酸结合寡聚化结构域样受体3(NALP3)、C反应蛋白(CRP)水平、视觉模拟评分(VAS)、关节活动评分低于治疗前,研究组低于对照组(P<0.05);血清SOD水平高于治疗前,研究组高于对照组(P<0.05)。结论二妙散加味联合医用臭氧局部灌注治疗能够显著减轻GA患者疼痛程度及炎症反应,促进患者临床症状缓解,进一步提高患者关节活动功能,临床疗效较好。Objective To investigate the effect of modified Ermiao powder combined with local perfusion of medical ozone on pain and joint range of motion in patients with gouty arthritis(GA).Methods A total of 88 patients with GA were divided into the control group and the study group according to random number table method,with 44 cases in each group.The control group was treated with colchicine tablets and diclofenac sodium enteric-coated capsules,and the study group was treated with modified Ermiao powder combined with medical ozone local perfusion therapy,both groups were treated for 3 days,and observed for 7 days after treatment.The curative effect after 3 days of treatment,the time of symptom relief,and pain degree,joint activity score,blood uric acid(BUA),erythrocyte sedimentation rate(ESR),alanine aminotransferase(ALT),Xanthine Oxidase(XOD),superoxide dismutase(SOD)and inflammatory response indicators before treatment and after 3 days of treatment were compared between the two groups.Results After 3 days of treatment,the total effective rate in the study group(93.18%,41/44)was higher than that in the control group(72.73%,32/44)(P<0.05);Compared with the control group,the time of symptom relief in the study group was shorter(P<0.05).After 3 days of treatment,the levels of peripheral blood ESR,serum 5-hydroxytryptamine(5-HT),prostaglandin E2(PGE2),BUA,XOD,cyclooxygenase-2(COX-2),nucleotide-binding oligomerization domain-like receptor 3(NALP3),C-reactive protein(CRP),and visual analogue scale(VAS)and joint activity score in both groups were lower than those before treatment,and the study group was lower than the control group(P<0.05);the levels of serum SOD were higher than those before treatment,and the study group was higher than the control group(P<0.05).Conclusion Modified Ermiao powder combined with medical ozone local perfusion therapy can significantly reduce the pain degree and inflammatory response of patients with GA,promote the relief of clinical symptoms,and further improve the joint function of patients.Th
