缺血性脑卒中居家康复效果评价指标的循证分析

摘　　要：目的:分析近年来国内外缺血性脑卒中居家康复效果评价指标的应用情况,为缺血性脑卒中居家康复效果评价指标体系的构建提供参考。方法:计算机检索the Cochrane Library、PubMed、Web of Science、CINAHL、EMbase、MedLine、EBSCO、Scopus、PsycINFO、中国知网、万方数据库、维普数据库、中国生物医学文献数据库等中英文数据库,对符合纳入及排除标准的文献进行循证评价,提取文献中相关评价指标的评价方法、评价工具或计算公式,并对提取的评价指标进行分类整理和分析。结果:共纳入49篇,文献发表于2002—2021年,其中中文文献8篇,英文文献41篇。提取包括生理功能、心理功能、社会功能、病人疾病应对水平、社会支持水平、医疗资源利用情况6个类别,共78项缺血性脑卒中居家康复效果评价指标。其中有49项指标引用频次≥3次,有2种及以上的评价方法、评价工具或计算公式的评价指标占比接近20%。结论:现有证据表明,缺血性脑卒中居家康复效果评价指标有待进一步拓展、规范和完善,关注病人除生理功能外如社会支持情况、医疗资源利用情况等其他方面的评价指标,借鉴循证分析的理念和方法建立系统、全面的缺血性脑卒中居家康复效果评价指标体系。Objective:To analyze the application of home rehabilitation evaluation index for ischemic stroke in recent years,and to provide reference for the establishment of home rehabilitation evaluation index system for ischemic stroke.Methods:The Cochrane Library,PubMed,Web of Science,CINAHL,EMbase,MedLine,EBSCO,Scopus,PsycINFO,CNKI,WanFang Database,VIP and other Chinese and English databases were searched.Articles meeting the inclusion and exclusion criteria were evaluated based on evidence,the evaluation methods,evaluation tools or calculation formulas of related evaluation indicators were extracted from the literature,and the extracted evaluation indicators were sorted out and analyzed.Results:A total of 49 articles were included,which were published from 2002 to 2021,including 8 in Chinese and 41 in English.A total of 6 categories including physiological function,psychological function,social function,patient′s disease coping level,social support level and medical resource utilization were extracted,and 78 evaluation indexes of home rehabilitation effect of ischemic stroke were extracted.Among them,49 indexes were cited more than 3 times,and evaluation indexes with 2 or more evaluation methods,evaluation tools or calculation formulas accounted for nearly 20%.Conclusion:At present,the evaluation indexes of the effects of ischemic stroke rehabilitation at home need to be further expanded,standardized and improved.The evaluation indexes of patients in addition to physiological function,such as social support and medical resource utilization,and other aspects should be paid attention to,and a systematic and comprehensive evaluation index system of the effects of ischemic stroke rehabilitation at home should be established by referring to the concept and methods of evidence-based analysis.
