七叶皂苷钠对胫骨平台骨折切开复位内固定术患者术后骨痂生长及血清VEGF水平的影响

摘　　要：目的探讨七叶皂苷钠对胫骨平台骨折切开复位内固定术患者术后骨痂生长及血清血管内皮生长因子(vascular endothelial growth factor,VEGF)水平的影响。方法选取医院2021年4月至2022年7月间收治的87例胫骨平台骨折患者,根据患者入院单尾号奇数纳对照组(44例)、偶数纳研究组(43例);两组均行切开复位内固定术治疗,对照组术后予常规治疗,研究组术后予七叶皂苷钠治疗;对比两组治疗效果、骨痂生长情况、血清VEGF水平、膝关节功能[膝关节功能评分(HSS)]及不良反应。结果研究组总优良率、骨痂密度、骨折断端边缘、骨痂量、骨痂边缘评分均较对照组高,差异有统计学意义(P<0.05);研究组治疗1个月VEGF较对照组高,且随访6个月HSS较对照组高,差异有统计学意义(P<0.05);两组间不良反应总发生率对比,差异无统计学意义(P>0.05)。结论胫骨平台骨折切开复位内固定术患者采用七叶皂苷钠治疗效果好,可促进术后骨痂生长,提高血清VEGF水平,改善膝关节功能,且安全性好。Objective To investigate the effects of aesculin sodium on callus growth and serum vascular endothelial growth factor(VEGF)levels in patients with tibial plateau fracture after open reduction and internal fixation.Methods A total of 87 patients with tibial plateau fractures admitted to the hospital from April 2021 to July 2022 were selected.According to the number of patients admitted to the hospital,the patients were divided into the odd-numbered control group(44 cases)and the even-numbered study group(43 cases).Both groups were treated with open reduction and internal fixation.The control group was treated with conventional treatment after surgery,and the study group was treated with sodium aesculin after surgery.The therapeutic effect,callus growth,serum VEGF level,knee function[knee function score(HSS)],and adverse reactions of the two groups were compared.Results The total good rate,callus density,fracture edge,callus volume,and callus edge score of the study group were higher than those of the control group,and the difference was statistically significant(P<0.05).VEGF expression in the study group was higher than that in the control group after 1 month of treatment,and HSS was higher than that in the control group after 6 months of follow-up,the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment effect of sodium aesculin in patients with tibial plateau fracture in open reduction and internal fixation is good,which can promote postoperative callus growth,increase the level of serum VEGF,and improve the function of the knee joint with good safety.
