摘　　要：目的 了解浙江省0-59岁健康人群风疹抗体水平。方法 采用分层随机抽样方法,选取浙江省11个地市0-59岁健康人群,采集血清标本,检测风疹IgG抗体,分析抗体阳性率和中位数浓度(Median concentration, MC)。结果 调查对象风疹IgG抗体总阳性率为72.30%(3 221/4 455)、MC为38.82IU/mL;0-7月龄、8-23月龄、2-4岁、5-9岁、10-14岁、15-19岁、20-29岁、30-39岁、40-59岁人群抗体阳性率分别为32.03%、85.00%、91.12%、66.90%、47.71%、71.64%、67.79%、74.72%、67.32%(趋势χ^(2)=193.91,P<0.001)。多因素Logistic回归分析显示,在8月龄-14岁儿童中,8-23月龄、2-4岁、5-9岁儿童风疹抗体阳性率高于10-14岁儿童[OR(95%CI):6.56(4.35-9.89)、11.09(8.09-15.19)、2.14(1.65-2.78)];1剂次、≥2剂次、不详含风疹成分疫苗(Rubella-containing vaccine, RCV)免疫史者高于无免疫史者[OR(95%CI):9.77(3.52-27.09)、8.19(2.87-23.35)、10.45(3.59-30.43)]。结论 2020年浙江省0-59岁人群风疹抗体水平总体较高,且随年龄增长呈下降趋势;需进一步加强中学生RCV接种工作。Objective To determine rubella antibody levels among healthy people 0-59 years of age in Zhejiang province.MethodsWe used a stratified random sampling method to select healthy people 0-59 years of age from 11 prefectures of Zhejiang.We tested sera for rubella IgG antibody to determine positivity rates and median concentrations(MC).ResultsThe overall positivity rate and MC of rubella IgG antibody were 72.30%(3221/4455)and 38.82 IU/m L,respectively.Positivity rates for 0-7-and 8-23-month-olds and 2-4-,5-9-,10-14-,15-19-,20-29-,30-39-,and 40-59-year-olds were 32.03%,85.00%,91.12%,66.90%,47.71%,71.64%,67.79%,74.72%,and 67.32%(trendχ^(2)=193.91,P<0.001).Multivariate logistic regression analysis restricted to children aged 8 months to 14 years showed that positivity rates were higher among 8-23-month-olds,2-4-year-olds,and 5-9-year-olds than among 10-14-year-olds[OR(95%CI):6.56(4.35-9.89)、11.09(8.09-15.19)、2.14(1.65-2.78)]and higher among subjects with 1-dose,≥2-dose,or unknown rubella-containing vaccine(RCV)vaccination history than among unvaccinated subjects[OR(95%CI):9.77(3.52-27.09)、8.19(2.87-23.35)、10.45(3.59-30.43)].ConclusionsRubella antibody levels were generally high among 0-59-year-old healthy people in Zhejiang in 2020 but declined with increasing age.We should further strengthen RCV vaccination for middle school students.
