摘 要:目的了解上海市戊型肝炎疫苗(Hepatitis E vaccine,HepE)接种的影响因素。方法选择42名各级专业人员和一般人员开展HepE接种影响因素的定性访谈,采用Colaizzi七步分析法、鱼骨图法和转化医学“4T”阶段模型,分析HepE接种影响因素及其随转化过程的变化。结果上海市HepE接种受认可度、知晓率、市场定位和现行免疫策略的影响。随着T1至T4转化过程,认可度的影响逐渐好转,市场定位和现行免疫策略的影响逐渐加重,知晓率的影响长期存在。共发现9个主要影响因素和32个次级影响因素,其中重要的主要影响因素包括普通人群知晓率低、非感染病/肝病专科人群知晓率低、健康教育传递渠道不通畅、定价策略待调整、支付方式待优化和现行非免疫规划疫苗免疫策略可完善。结论本调查发现上海市HepE接种受多方面因素影响,为优化HepE免疫策略提供了依据。Objective To explore factors influencing hepatitis E vaccine (HepE) vaccination.MethodsWe selected 42 professional experts and members of the general population for a qualitative interview. We used Colaizzi 7 - step phenomenological analysis, fishbone diagrams, and the " 4T" stage model of translational medicine theory to determine factors influencing HepE vaccination and the influence changing with translational process.ResultsHepE vaccination in Shanghai is influenced by recognition, awareness, market positioning, and current immunization strategy. With translational process from T1 to T4, the influence of recognition becomes lower while the impact of market positioning or current immunization strategy becomes greater and the impact of awareness endures for a long time. A total of 9 primary influencing factors and 32 secondary influencing factors were identified. Important primary factors included awareness in the general population and in medical workers, health knowledge delivery, pricing strategy, payment mechanism, and current immunization strategy for non-Expanded Program on Immunization vaccines.ConclusionsMultiple factors influence hepatitis E vaccination in Shanghai, which may provide evidence for optimizing hepatitis E immunization strategy.
