HIV感染孕妇的心理应激状况及其影响因素分析

摘　　要：目的分析人类免疫缺陷病毒(HIV)感染孕妇的心理应激状况及其影响因素,为HIV感染孕妇提供更好的心理干预。方法选取2019年5月—2022年1月广州医科大学附属市八医院收治的HIV感染孕妇300例作为研究对象,分析孕妇的心理应激状况,并分析心理应激与医学应对方式、社会支持和生命质量之间的相关性。结果300例患者的平均情绪/存在压力为(12.97±4.66)分,平均社会压力为(14.68±4.32)分,平均工具性压力为(5.95±4.08)分,平均SDS评分为(42.28±9.25)分,平均SAS评分为(46.45±5.35)分。通过屈服策略预测,SS-HIV评分的β值为0.514,P值为0.021;SDS评分的β值为0.416,P值为0.031;SAS评分β值为0.358,P值为0.025。通过支持利用度预测,SS-HIV评分的β值为-0.421,P值为0.015;SDS评分的β值为-0.431,P值为0.021;SAS评分β值为-0.349,P值为0.032。通过生命质量量表预测,SS-HIV评分的β值为-0.241,P值为0.016;SDS评分的β值为-0.341,P值为0.018;SAS评分β值为-0.141,P值为0.028。结论HIV感染孕妇存在不同程度的心理应激状况,在临床上可以通过医学应对方式、社会支持方式和生命质量预测心理状态,从而采取针对性措施以减轻患者疾病压力,缓解不良情绪。Objective To analyze the psychological stress status and its influencing factors of pregnant women infected with human immunodeficiency virus(HIV),in order to provide better psychological intervention for pregnant women infected with HIV.Methods A total of 300 pregnant women infected with HIV admitted to the Eighth Affiliated Hospital of Guangzhou Medical University from May 2019 to January2022 were selected as the study subjects.The psychological stress status of pregnant women were analyzed,and the correlation among psychological stress and medical coping styles,social support,and quality of life were analyzed.Results The average emotional/existential stress of 300 patients was(12.97±4.66)points,the average social stress was(14.68±4.32)points,the average instrumental stress was(5.95±4.08)points,the average SDS score was(42.28±9.25)points and the average SAS score was(46.45±5.35)points.Through yield strategies prediction,theβvalue of SS-HIV score was 0.514 and the P value was0.021;theβvalue of SDS score was 0.416 and the P value was 0.031;theβvalue of SAS score was 0.358and the P value was 0.025.Through supporting utilization prediction,theβvalue of SS-HIV score was-0.421,and the P value was 0.015;theβvalue of SDS score was-0.431 and the P value was 0.021;theβvalue of SAS score was-0.349 and the P value was 0.032.Through the quality of life scale prediction,theβvalue of SS-HIV score was-0.241,and the P value was 0.016;theβvalue of SDS score was-0.341 and the P value was 0.018;theβvalue of SAS score was-0.141 and the P value was 0.028.Conclusion Pregnant women infected with HIV had different degrees of psychological stress status.In clinical practice,the psychological state could be predicted through medical coping style,social support style and quality of life,in order to take targeted measures to reduce the disease stress and alleviate the bad mood of patients.
