CT灌注成像诊断急性脑梗死的研究进展

摘　　要：急性脑梗死患者早期诊断十分重要,临床常采取CT进行诊断。CT灌注成像属于一种先进的影像技术,可反映患者器官功能,用于急性脑梗死患者中,可有效反映急性脑梗死患者局部组织血流灌注量的变化,其参数可区分潜在的可逆性缺血半暗带与不可逆损伤梗死核心组织,为临床溶栓治疗提供依据。此外,CT灌注成像可较好观察急性脑梗死患者脑组织侧支循环状态,可为是否进行血管内支架植入手术治疗提供指导,避免血管内支架植入术并发症发生,保障患者安全。目前,临床多项研究均指出急性脑梗死患者应用CT灌注成像取得了较高诊断价值,可较好反映患者疾病情况,其应用前景广阔,且该诊断安全可靠,对患者影响较小。早期诊断急性脑梗死是患者早期治疗及预后的关键,需引起临床重视。本文从CT灌注成像原理、诊断急性脑梗死的意义和局限性及应用价值进行综述,以期为急性脑梗死的临床诊断提供指导。Early diagnosis of acute cerebral infarction is very important,and CT is often used in clinical diagnosis.CT perfusion imaging is an advanced imaging technology,which can reflect the function of patients'organs.It is used in patients with acute cerebral infarction,which can effectively reflect the changes of local tissue blood perfusion volume in patients with acute cerebral infarction.Its parameters can distinguish the potential reversible ischemic penumbra and irreversible injury of the core tissue of infarction,providing a basis for clinical thrombolytic therapy.In addition,CT perfusion imaging can better observe the collateral circulation status of brain tissue in patients with acute cerebral infarction,and provide guidance for whether to carry out intravascular stent implantation surgery,avoid the complications of intravascular stent implantation surgery,and ensure the safety of patients.At present,many clinical studies have pointed out that the application of CT perfusion imaging in patients with acute cerebral infarction has achieved high diagnostic value,can better reflect the disease situation of patients,its application prospect is broad,and the diagnosis is safe and reliable,and has little impact on patients.The early diagnosis of acute cerebral infarction is the key to the early treatment and prognosis of patients,and clinical attention should be paid to it.This article reviews the principles,significance and limitations in the diagnosis of acute cerebral infarction,and application value of CT perfusion imaging,in order to provide guidance for the clinical diagnosis of acute cerebral infarction.
