摘 要:脓毒症是一类异质性疾病,其病因、发病机理以及临床转归等具有明显的复杂性和多样性,并且有较高的发病率,其引发的脓毒症休克具有较高的致死率。因此此类疾病的早期诊断与治疗是临床面临的重大挑战。液体治疗是指通过补充或限制特定种类液体从而维持体液平衡的治疗方法,广泛应用于多学科疾病的治疗。关于脓毒症休克的基础及临床研究表明,早期合理的液体复苏策略作为脓毒症治疗的重要环节,是改善脓毒症休克预后的基础。随着近年来脓毒症和液体复苏治疗相关的临床指南和专家共识不断推陈出新,探讨符合当今医疗时代的脓毒症休克液体复苏治疗方案是一项十分迫切的任务。本文从脓毒症休克的血流动力学变化机制和液体复苏治疗原则等,对脓毒症休克患者液体复苏的近期相关研究进展进行归纳总结并进行综述,以期为此类患者的临床救治提供依据。Sepsis is a heterogeneous disease. Its etiology, pathogenesis and clinical outcomes are characterized by significant complexity and diversity,and it has a high incidence rate. Septic shock caused by sepsis has a high mortality rate. Therefore,the early diagnosis and treatment of such diseases are significant clinical challenges. Liquid therapy refers to a treatment method that maintains fluid balance in the body by supplementing or limiting specific types of fluids and is widely used in the treatment of multidisciplinary diseases. Basic and clinical studies on septic shock have shown that early and reasonable fluid resuscitation strategies,as an important link in the treatment of sepsis,are the foundation of improving the prognosis of septic shock. With the emergence of related clinical guidelines and expert consensus of sepsis and fluid resuscitation treatment in recent years,exploring fluid resuscitation treatment plans for sepsis shock that are in line with the current medical era is an urgent task. This article reviews recent research progress on fluid resuscitation in septic shock from the perspectives of hemodynamic changes and principles of fluid resuscitation and so on,in order to provide a basis for clinical treatment.
