心脏彩超在多病因慢性心力衰竭诊断中的临床应用研究

摘　　要：目的分析疑似多病因慢性心力衰竭受检者采取心脏彩超检查对敏感度、特异性的影响。方法将郑州大学第一附属医院2021年1月—2021年12月收治的113例疑似多病因慢性心力衰竭的受检者作为研究对象,均接受常规心电图检查、动态心电图检查、心脏彩超检查,以病理结果为金标准,对受检者常规心电图、动态心电图、心脏彩超的诊断灵敏度、特异性、准确率、阴性预测值、阳性预测值进行比较。结果113例疑似多病因慢性心力衰竭患者中,共有76例确诊,动态心电图与心脏彩超比较,灵敏度、特异性、准确率、阴性预测值、阳性预测值无差异,P>0.05;动态心电图与心脏彩超的灵敏度、特异性、准确率、阴性预测值、阳性预测值均高于常规心电图,P<0.05;慢性心力衰竭的LVEF、E/A值低于非慢性心力衰竭,Tei指数、LVESV、LVEDV高于非慢性心力衰竭,P<0.05。结论心脏彩超检查有着良好诊断效果,其特异性与敏感度较高,可以考虑积极推广。Objective To analyze the influence of color doppler echocardiography on sensitivity and specificity of pa-tients with suspected multi-cause chronic heart failure.Methods Totally 113 patients with suspected chronic heart failure of multiple causes accepted in our hospital from January 2021 to December 2021 were selected as the study sub-jects.They all received routine electrocardiogram examination,dynamic electrocardiogram examination and color doppler echocardiography,with pathological results as the gold standard.The diagnostic sensitivity,specificity,accuracy,nega-tive predictive value and positive predictive value of routine electrocardiogram,dynamic electrocardiogram and color doppler echocardiography were compared.ResultsAmong 113 patients with suspected chronic heart failure of multiple causes,76 cases were confirmed.There were no differences in sensitivity,specificity,accuracy,negative predictive val-ue and positive predictive value between dynamic electrocardiogram and color doppler echocardiography(P>0.05).The sensitivity,specificity,accuracy,negative predictive value and positive predictive value of dynamic electrocardio-gram and cardiac color ultrasound were higher than those of conventional electrocardiogram,P<0.05;The LVEF and E/A values of chronic heart failure were lower than those of non-chronic heart failure,while Tei index,LVESV and LVEDV were higher than those of non-chronic heart failure,P<0.05.Conclusion Color doppler echocardiography has a good diagnostic effect with high specificity and sensitivity.It is worthy of active promotion.
