中西医结合治疗心血瘀阻型胸痹临床疗效及对FBI、BV和PV的影响

摘　　要：目的探讨中西医结合治疗在心血瘀阻型胸痹中的应用效果及对血液流变学各指标的影响。方法通过对2019年2月至2021年5月商丘市中医院收治的132例心血瘀阻型胸痹患者进行随机分组,分为对照组和实验组各66例,对照组患者给予西药治疗,单硝酸异山梨酯片、曲美他嗪片、阿司匹林肠溶片、阿托伐他汀钙片口服,硝酸甘油注射液5 mg溶于5%葡萄糖注射液500 mL,静脉滴注,每日1次,连续治疗2周。实验组在此基础上加用血府逐瘀汤中医辨证治疗2周。观察比较两组治疗效果及血液流变学指标差异。结果实验组总有效率(95.45%)明显高于对照组(71.21%),对比差异有统计学意义(P<0.05);实验组纤维蛋白原(FBI)、全血黏度(BV)、血浆黏度(PV)分别为(3.13±0.70)g/L、(3.39±0.59)mPa/s、(1.50±0.60)mPa/s,较对照组明显下降,对比差异有统计学意义(P<0.05)。结论针对心血瘀阻型胸痹患者应用血府逐瘀汤辨证治疗效果显著,可有效缓解心绞痛,改善血液流变学指标,值得推广应用。Objective To investigate the application effect of integrated Chinese and western medicine in the treatment of chest obstruction of heart blood stasis and the influence on the indexes of hemorheology.Methods A total of 132 pa-tients with chest arthralgia of heart blood stasis type admitted to our hospital from February 2019 to May 2021 were ran-domly divided into control group and experimental group with 66 patients in each group.Patients in the control group were treated with western medicine,isosorbide mononitrate tablets,trimetazidine tablets,aspirin enteric-coated tablets and atorvastatin calcium tablets orally.Nitroglycerin injection 5mg dissolved in 5%glucose injection 500 mL intrave-nously,once a day,for 2 weeks.The experimental group was additionally treated with Xuefu Zhuyu decoction for 2 weeks.The therapeutic effect and hemorheology indexes of the two groups were observed and compared.Results sThe total effective rate of the experimental group(95.45%)was significantly higher than that of the control group(71.21%),and the difference was statistically significant(P<0.05).The fibrinogen(FBI),whole blood viscosity(BV)and plasma viscosity(PV)of the experimental group were(3.13±0.70)g/L,(3.39±0.59)mPa/s and(1.500.60)mPa/s,respectively,which decreased significantly compared with the control group,and the difference was statistically significant(P<0.05).Conclusion The application of Xuefu Zhuyu decoction in the treatment of pa-tients with thoracic obstruction of heart blood stasis has remarkable effect,which can effectively relieve angina pectoris and improve hemorheology indexes,and is worth popularizing and applying.
