化毒除湿汤加味联合常规抗生素治疗急性盆腔炎湿毒壅盛证的临床研究

摘　　要：目的:探讨化毒除湿汤加味联合常规抗生素治疗急性盆腔炎(APID)湿毒壅盛证的疗效。方法:选取2019年7月—2022年7月在常州市中医医院就诊的APID湿毒壅盛证女性患者80例,按照随机数字表法分为对照组和治疗组,每组40例。对照组采取常规抗生素治疗;治疗组在对照组基础上采取化毒除湿汤加味内服。两组均治疗2周。比较两组APID湿毒壅盛证患者的主要症状消失时间、湿毒壅盛证单项症状评分、临床疗效以及血清C-反应蛋白(CRP)、降钙素原(PCT)、血清肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)水平。结果:治疗组的腹痛、体温异常、白带异常消失时间明显短于对照组(P<0.01);治疗后,两组湿毒壅盛证单项症状评分明显下降(P<0.01),且治疗组评分明显低于对照组(P<0.05);治疗组总有效率为95.00%(38/40)较对照组75.00%(30/40)明显升高(P<0.05);治疗后,两组血清CRP与PCT水平显著降低(P<0.01),且治疗组明显低于对照组(P<0.05);治疗后,两组血清TNF-α和IL-6水平显著降低(P<0.01),且治疗组与对照组相比降低更明显(P<0.05,P<0.01)。结论:化毒除湿汤加味联合常规抗生素治疗APID湿毒壅盛证有利于改善患者症状体征与中医证候,减轻炎症反应,提高治疗效果。Objective:To investigate the efficacy of Huadu Chushi Decoction combined with conventional antibiotics in treating acute pelvic inflammatory disease(APID)of dampness-toxin accumulation syndrome.Methods:A total of 80 female APID patients with syndrome of dampness-toxin accumulation were randomly divided into the control group and the treatment group,with 40 patients in each group.The control group was treated with conventional antibiotics;on which basis,the treatment group was also treated with Huadu Chushi Decoction.Both groups were treated for 2 weeks.Main symptoms disappearance time,individual symptom score,clinical efficacy,and serum levels of C-reactive protein(CRP),procalcitonin(PCT),tumor necrosis factorα(TNF-α)and interleukin 6(IL-6)were compared between the two groups.Results:The disappearance time of abdominal pain,abnormal body temperature and abnormal leucorrhea was significantly shorter in the treatment group than that in the control group(P<0.01).The scores of individual symptoms of dampness-toxin accumulation syndrome were significantly decreased in both groups after the treatment(P<0.05);the scores of individual symptoms were significantly lower in the treatment group than those in the control group(P<0.05).The total effective rate of the treatment group was 95.00%(38/40),which was significantly higher than 75.00%(30/40)of the control group(P<0.05).The serum levels of CRP and PCT were significantly decreased in both groups after the treatment(P<0.01);which were significantly lower in the treatment group than those in the control group(P<0.05).The serum levels of TNF-αand IL-6 were significantly decreased in both groups after the treatment(P<0.01);which were lower in the treatment group than those in the control group(P<0.05,P<0.01).Conclusion:Huadu Chushi Decoction combined with conventional antibiotics is beneficial to improving symptoms,signs and TCM syndromes,reducing inflammation and improving therapeutic effect in treatment of APID of dampness-toxin accumulation syndrome.
