产后集束泌乳干预对初产妇泌乳时间、纯母乳喂养率及自我护理能力的影响

摘　　要：目的:探讨产后集束泌乳干预对初产妇泌乳时间、纯母乳喂养率及自我护理能力的影响。方法:选择2021年5月至2022年5月在医院分娩的初产妇100例,以随机数表法分为两组,对照组(n=50)常规护理;观察组(n=50)在常规护理基础上给予产后集束泌乳干预。观察初产妇泌乳情况,统计纯母乳喂养率,并评估初产妇的自我护理能力。结果:观察组初产妇首次泌乳时间(1.02±0.21)d,短于对照组,产后1周的乳汁充足率为92.00%,高于对照组(P<0.05)。观察组初产妇产后42d的纯母乳喂养率为88.00%,高于对照组(P<0.05)。干预后观察组初产妇喂养意愿、哺乳技能、乳房护理、婴儿护理、产后保健与自我护理总评分分别为(18.62±1.83)分、(18.68±1.87)分、(17.93±1.79)分、(19.52±1.90)分、(18.99±1.86)分、(18.75±1.74)分,均高于对照组(P<0.05)。结论:对初产妇产后采用产后集束泌乳干预,能缩短初产妇泌乳时间并增加泌乳量,提高纯母乳喂养率且增强初产妇的自我护理能力。Objective:To explore the influence of postpartum cluster lactation intervention on lactation time,exclusive breastfeeding rate and self-care ability of primiparas.Methods:100 primiparas who gave birth in the hospital from May 2021 to May 2022 were randomly divided into two groups,the control group(n=50)was given routine care;The observation group(n=50)was given postpartum cluster lactation intervention on the basis of routine nursing.Observe the lactation of primiparas,count the exclusive breastfeeding rate,and evaluate the self-care ability of primiparas.Results:The first lactation time of primipara in the observation group was(1.020.21)d,which was shorter than that in the control group,and the milk adequacy ratio was 92.00%one week after delivery,which was higher than that in the control group(P<0.05).The exclusive breastfeeding rate of primiparas in the observation group was 88.00%42 days after delivery,which was higher than that in the control group(P<0.05).After the intervention,the total scores of primiparas'feeding willingness,nursing skills,breast care,infant care,postpartum care and self-care in the observation group were(18.62±1.83),(18.68±1.87),(17.93±1.79),(19.52±1.90)and(18.99±1.86)respectively.Conclusion:The postpartum cluster lactation intervention for primiparas can shorten the lactation time and increase the lactation quantity,improve the exclusive breastfeeding rate and enhance the self-care ability of primiparas.
