电子胃镜和超声内镜应用于上消化道黏膜下病变的诊断价值分析

摘　　要：目的探讨电子胃镜和超声内镜应用于上消化道黏膜下病变的诊断价值。方法选取2020年2月—2022年5月枣庄市中医医院的47例疑似上消化道黏膜下病变患者为研究对象。所有患者均展开普通电子胃镜与超声内镜检查操作,将病理活检结果作为金标准,对比两种检查方法的诊断结果。结果病理活检属于异位胰腺患者5例(10.64%),属于胃间质瘤患者12例(25.53%),属于平滑肌瘤患者20例(42.55%),属于脂肪瘤患者3例(6.38%),属于血管瘤患者1例(2.13%),属于囊肿患者6例(12.77%)。超声内镜疾病符合率高于电子胃镜总符合率,差异有统计学意义(P<0.05)。超声内镜检查敏感度以及准确度均高于电子胃镜,差异有统计学意义(P<0.05)。结论在对上消化道黏膜下病变患者实施临床诊断期间,合理选择超声内镜检查方法,可显著提高诊断敏感度以及准确度。Objective To investigate the diagnostic value of electronic gastroscopy and ultrasonic endoscopy applied to upper gastrointestinal submucosal lesions.Methods From February 2020 to May 2022,47 patients with suspected upper gastrointestinal submucosal lesions in Zaozhuang City Hospital of Traditional Chinese Medicine were selected as the research objects.All patients underwent conventional electronic gastroscopy and endoscopic ultrasound examination,used pathological biopsy results as the gold standard,and compared the diagnostic results of the two examination methods.Results Pathological biopsy was performed in 5 patients(10.64%)with ectopic pancreas,12 patients(25.53%)belonged to gastric mesenchymal stromal tumor,20 patients(42.55%)belonged to smooth muscle tumor,3 patients(6.38%)belonged to lipoma,1 patient(2.13%)belonged to hemangioma,and 6 patients(12.77%)belonged to cyst.The coincidence rate of endoscopic ultrasound for diseases was higher than the total coincidence rate of electronic gastroscopy,and the difference was statistically significant(P<0.05).The sensitivity and accuracy of endoscopic ultrasound examination were higher than those of electronic gastroscopy,and the difference was statistically significant(P<0.05).Conclusion Clinical diagnosis of patients with upper gastrointestinal submucosal lesions,reasonable choice of ultrasonic endoscopy methods can significantly improve the sensitivity and accuracy of diagnosis.
