恩替卡韦联合乙肝解毒汤治疗乙型肝炎合并肺结核患者的临床效果探讨

摘　　要：目的探讨乙型肝炎+肺结核患者治疗过程中应用恩替卡韦联合乙肝解毒汤的作用效果。方法选取2019年1月—2022年12月研究期间广西河池市人民医院治疗的40例乙型肝炎+肺结核患者进行研究,通过数据随机法将其分为西药治疗组(20例,常规治疗+恩替卡韦)与中西结合组(20例,常规治疗+恩替卡韦+乙肝解毒汤),对比两组实验室指标、症状评分、治疗效果、外周血淋巴细胞亚群情况与不良反应情况。结果治疗后,中西结合组患者谷丙转氨酶、谷草转氨酶、乙肝病毒脱氧核糖核酸指标低于西药治疗组,血清总胆红素高于西药治疗组,症状评分低于西药治疗组,治疗效果高于西药治疗组,CD4^(+)高于西药治疗组,CD8^(+)低于西药治疗组,差异有统计学意义(P<0.05)。治疗后,中西结合组患者不良反应情况发生率为0低于西药治疗组的30.00%,差异有统计学意义(χ^(2)=4.902,P<0.05)。结论患者在常规肺结核用药基础上给予恩替卡韦联合乙肝解毒汤治疗,能够有效改善患者体内炎性状况,优化实验室指标,加快症状改善,促使肺结核病灶快速吸收,并且用药安全性高。Objective To explore the effect of applying entecavir combined with hepatitis B detoxification Decoction during the treatment of patients with hepatitis B+pulmonary tuberculosis.Methods 40 cases of hepatitis B+pulmonary tuberculosis patients treated by the People's Hospital of Hechi City,Guangxi Zhuang Autonomous Region during the study period from January 2019 to December 2022 were selected for the study.They were randomly divided into a western medicine treatment group(20 cases,conventional treatment+entecavir)and a combination of traditional Chinese and western medicine group(20 cases,conventional treatment+entecavir+hepatitis B detoxification decoction).The laboratory indexes,symptom scores,treatment effects,peripheral blood lymphocyte subsets and adverse reactions of the two groups were compared.Results After treatment,the alanine transaminase,aspartate transaminase,and deoxyribonucleic acid of hepatitis B virus,indicators of patients in the combination of traditional Chinese and western medicine group were lower than those in the western medicine treatment group,while TBIL was higher than that in the western medicine treatment group,the symptom score was lower than that in the western medicine treatment group,and the treatment effect was higher than that in the western medicine treatment group,CD4^(+)was higher than that in the western medicine treatment group,and CD8^(+)was lower than that in the western medicine treatment group,the difference was statistically significant(P<0.05).After treatment,the incidence of adverse reactions in the combination of traditional Chinese and Western medicine group was 0 lower than that in the Western medicine treatment group of 30.00%,the difference was statistically significant(χ^(2)=4.902,P<0.05).Conclusion Patients were given entecavir combined with hepatitis B detoxification Decoction on the basis of conventional tuberculosis medication,which can effectively improve the inflammatory status of the patient's body,optimize the laboratory indexes,accelerate the improveme
