摘 要:目的探讨多层螺旋CT与MRI在腰椎压缩性骨折诊断中的价值。方法选取镇江三五九医院影像科2017年1月—2022年12月收治疑似腰椎压缩性骨折患者70例,均予多层螺旋CT与核磁共振成像(magnetic resonance imaging,MRI)检查,以临床综合诊断为金标准,比较两种技术诊断效能及分型诊断准确性,另根据骨折损伤时间将患者分为新鲜骨折组与陈旧骨折组,比较两组MRI征象的差异。结果MRI诊断符合率为98.57%、灵敏度为98.57%,高于CT检查的88.57%、88.57%,差异有统计学意义(χ^(2)=5.143、5.143,P<0.05)。MRI、CT分型诊断准确率为98.57%、85.71%。MRI下陈旧性骨折组椎体滑脱率、骨小梁结构紊乱率、椎间盘变性与真空征率,均高于新鲜骨折组,骨折线清晰率、脊柱损伤出血率、脊柱旁软组织影率低于新鲜性骨折组,差异有统计学意义(P<0.05)。结论MRI对腰椎压缩性骨折诊断效能以及分型诊断准确率比多层螺旋CT高,且有利于识别新鲜与陈旧性骨折。Objective To investigate the value of multi-slice spiral CT and MRI in the diagnosis of lumbar compression fractures.Methods From January 2017 to December 2022,70 patients with suspected lumbar Compressibility fractures were selected from the imaging department of Zhenjiang 359 Hospital,and all of them were examined with multi-slice spiral CT and magnetic resonance imaging(MRI),used clinical comprehensive diagnosis as the gold standard,compared the diagnostic efficacy and classification accuracy of the two techniques.Additionally,divided patients into fresh fracture group and old fracture group based on the time of fracture injury,and compared the differences in MRI signs between the two groups.Results The diagnostic accuracy and sensitivity of MRI were 98.57%and 98.57%,respectively,which were higher than those of CT examination(88.57%and 88.57%,respectively),the difference was statistically significant(χ^(2)=5.143,5.143,P<0.05).The diagnostic accuracy of MRI and CT classification was 98.57%and 85.71%,respectively.The rate of vertebral detachment,bone trabecular structure disorder,intervertebral disc degeneration,and vacuum sign in the old fracture group under MRI were higher than those in the fresh fracture group,the clarity rate of fracture lines,bleeding rate of spinal injuries,and soft tissue shadow rate near the spine were lower than those in the fresh fracture group,the difference was statistically significant(P<0.05).Conclusion The diagnostic efficiency and classification accuracy of MRI for lumbar compression fractures are higher than that of multi-slice spiral CT,and it is beneficial to identify fresh and old fractures.
