经阴道彩色多普勒超声诊断剖宫产切口瘢痕妊娠的价值分析

摘　　要：目的探讨对剖宫产切口瘢痕妊娠孕妇给予经阴道彩色多普勒超声诊断后获得临床效果。方法选取2020年3月—2022年5月枣庄市立医院74例疑似剖宫产切口瘢痕妊娠孕妇作为研究对象。临床针对同一批剖宫产切口瘢痕妊娠孕妇合理展开经腹部超声诊断以及经阴道彩色多普勒超声诊断,在将最终临床综合诊断结果作为金标准前提下,对两种方法诊断剖宫产切口瘢痕妊娠的诊断效能等展开对比。结果所有疑似剖宫产切口瘢痕妊娠孕妇经临床综合诊断后,属于剖宫产切口瘢痕妊娠孕妇60例,不属于剖宫产切口瘢痕妊娠孕妇14例。经阴道彩色多普勒超声诊断疑似剖宫产切口瘢痕妊娠的特异度85.71%(12/14)、敏感度98.33%(59/60)、准确度95.95%(71/74),均高于经腹部超声诊断疑似剖宫产切口瘢痕妊娠的特异度28.57%(4/14)、敏感度66.67%(40/60)、准确度59.46%(44/74),差异有统计学意义(χ^(2)=8.100、17.053、25.037,P<0.05)。结论临床针对剖宫产切口瘢痕妊娠实施诊断期间,经腹部彩色多普勒超声诊断以及经阴道彩色多普勒超声均获得广泛应用。同经腹部比较,经阴道方式的有效应用获得的诊断效能较高。Objective To explore the clinical effect of transvaginal color Doppler ultrasound in the diagnosis of pregnant women with cesarean section incision scar.Methods A total of seventy-four suspected pregnant women with cesarean section incision scar from March 2020 to May 2022 in Zaozhuang Municipal Hospital were selected as the study objects.In clinical practice,transabdominal ultrasound and transvaginal color Doppler ultrasound were carried out reasonably for the same group of pregnant women with scar incision during cesarean section.On the premise of taking the final comprehensive clinical diagnosis result as the gold standard,the diagnostic efficacy of the two methods for the diagnosis of cesarean section incision scar pregnancy was compared.Results After comprehensive clinical diagnosis of all suspected cesarean incision scar pregnancies,there were 60 cases of pregnant women with cesarean incision scar and 14 cases of pregnant women with noncesarean incision scar.The specificity 85.71%(12/14),sensitivity 98.33%(59/60)and accuracy 95.95%(71/74)of transvaginal color Doppler ultrasound in the diagnosis of suspected cesarean incision scar pregnancy were higher than those of transabdominal ultrasoundy in the diagnosis of suspected cesarean incision scar pregnancy 28.57%(4/14),sensitivity 66.67%(40/60),accuracy 59.46%(44/74),and the difference was statistically significant(χ^(2)=8.100,17.053,25.037,P<0.05).Conclusion Both transabdominal color Doppler ultrasound and transvaginal color Doppler ultrasound have been widely used in the diagnosis of cesarean section incision scar pregnancy.Compared with transabdominal,the effective application of transvaginal method obtains higher diagnostic efficiency.
