分析心电图诊断冠心病不稳定型心绞痛的效果

摘　　要：目的分析心电图对于冠心病不稳定性心绞痛的临床诊断效果与价值。方法选取2021年9月—2022年9月菏泽市牡丹人民医院接诊的疑似冠心病不稳定型心绞痛患者97例参与研究,患者均需开展动态心电图检查,以冠脉造影诊断结果为金标准,计算动态心电图检查对冠心病不稳定型心绞痛的诊断效能,并统计动态心电图检查方法诊断冠脉造影金标准的一致性。结果冠脉造影诊断冠心病不稳定型心绞痛阳性68例,动态心电图诊断冠心病不稳定型心绞痛阳性69例,动态心电图对冠心病不稳定型心绞痛的诊断准确度92.78%,灵敏度95.59%,特异度86.21%,阳性预测值94.20%,阴性预测值89.29%。动态心电图诊断冠心病不稳定型心绞痛与冠脉造影金标准的一致性良好(Kappa=0.826,P<0.05)。结论动态心电图对冠心病不稳定型心绞痛的诊断效果与冠脉造影金标准的诊断结果相近,临床应用效果更确切,值得推广。Objective To analyze the clinical diagnosis effect and value of electrocardiogram in unstable angina pectoris of coronary heart disease.Methods A total of ninety-seven patients with suspected coronary heart disease unstable angina pectoris were randomly selected from the Mudan People's Hospital from September 2021 to September 2022 to participate in the study.All the suspected patients in this group need to carry out holter electrocardiogram examination,and the diagnostic efficacy of holter electrocardiogram examination in the diagnosis of unstable angina pectoris of coronary heart disease was calculated with the diagnosis result of coronary angiography as the gold standard.The consistency of dynamic electrocardiogram in the diagnosis with the gold standard of coronary angiography was analyzed.Results The positive rate of unstable angina pectoris was 68 cases diagnosed by coronary angiography and 69 cases diagnosed by dynamic electrocardiography.The diagnostic efficacy indexes of holter electrocardiogram for unstable angina pectoris were:accuracy 92.78%,sensitivity 95.59%,specificity 86.21%,positive predictive value 94.20%,negative predictive value 89.29%.The holter electrocardiogram diagnosis of unstable angina pectoris of coronary heart disease was in good agreement with the gold standard of coronary angiography(Kappa=0.826,P<0.05).Conclusion The diagnostic effect of holter electrocardiogram on unstable angina pectoris of coronary heart disease is similar to the gold standard of coronary angiography,so the clinical application effect is more accurate and worthy of promotion.
