摘 要:目的分析乙型肝炎病毒(HBV)相关性肝病患者循环血可溶性程序性死亡因子(sPD-1)、CXCL-10、CXCL-11水平表达差异及意义。方法采用酶联免疫法检测sPD-1,CXCL-10、CXCL-11,运用实时荧光定量PCR法检测HBV-DNA载量,采用化学发光法检测乙肝病毒感染标志物,及运用全自动生化分析仪测定肝功能指标。结果健康对照组外周血sPD-1、CXCL-10、CXCL-11表达水平最低,与HBV携带者组比较差异无统计学意义(P>0.05);其他各组与对照组及HBV携带者组比较差异均有统计学意义(P<0.05)。随着病情进展,外周血sPD-1、CXCL-10、CXCL-11表达水平明显升高,在重型乙型肝炎组最高,与其他各组比较差异有统计学意义(P<0.05);HBV相关性肝病循环血中总体sPD-1表达水平与HBV-DNA载量、ALT、AST表达水平均呈正相关(P<0.05);HBV相关性肝病循环血中总体CXCL-10、CXCL-11表达水平与HBV-DNA载量、ALT、AST表达水平亦呈正相关(P<0.05);HBV相关性肝病各组循环血中sPD-1表达水平与CXCL-10、CXCL-11表达水平均呈正相关。同时各组循环血中CXCL-10表达水平与CXCL-11表达水平亦呈正相关。结论sPD-1、CXCL-10、CXCL-11表达水平可能成为潜在反应肝组织损伤、病毒复制水平、病情变化、治疗新靶点的指标和方向。Objective To analyze difference and significance of soluble programmed death factor(sPD-1),CXCL-10,CXCL-11 in circulating blood of patients with hepatitis B virus(HBV)associated liver disease.Methods sPD-1,CXCL-10,CXCL-11 were detected by ELISA,HBV DNA load was detected by real-time fluorescent quantitative PCR,hepatitis B virus infection markers were detected by chemiluminescence,and liver function indexes were measured by automatic biochemical analyzer.Results The sPD-1,CXCL-10,CXCL-11 in peripheral blood of healthy control group were the lowest,but there was no significant difference compared with HBV carrier group(P>0.05).There was significant difference between other groups and control group and HBV carrier group(P<0.05).With the progression of the disease,peripheral blood sPD-1,CXCL-10 and CXCL-11 were significantly increased,which were the highest in the severe hepatitis B group,with a statistically significant difference compared with other groups(P<0.05).The total sPD-1 in circulating blood of HBV associated liver disease was positively correlated with HBV DNA load,ALT,AST(P<0.05).The CXCL-10 and CXCL-11 in circulating blood of HBV associated liver disease was also positively correlated with HBV DNA load,ALT,AST(P<0.05).The sPD-1 in circulating blood of each group with HBV related liver disease was positively correlated with CXCL-10 and CXCL-11.At the same time,the CXCL-10 and CXCL-11 in circulating blood of each group also showed a positive correlation.Conclusion The sPD-1,CXCL-10 and CXCL-11 may be indicators and directions of potential reaction to liver tissue injury,virus replication,disease changes and new therapeutic targets.
