摘  要:目的 比较改良TAP技术和DK-CRUSH技术在治疗冠状动脉真性分叉病变上的疗效。方法 纳入2017年9月至2021年3月在我院就医的冠状动脉真性分叉病变患者,随机分为改良TAP组及DK-CRUSH组,比较两组间在主要研究终点(术后9个月复查冠状动脉造影支架内再狭窄、术后1年患者主要心血管不良事件、支架内血栓)及次要研究终点(PCI相关心肌梗死、X线暴光时间、PCI成功率、对比剂用量)上的差异。结果 两组间在主要终点事件、PCI相关心肌梗死及手术成功率上差异无统计学意义(P>0.05),但改良TAP组的X线暴露时间、对比剂用量明显低于DK-CRUSH组(P<0.05)。结论 改良TAP技术将TAP技术和JBT技术相结合,在保留TAP简便、降低费用等优点的同时,改良TAP术式与DK-CRUSH术式疗效相当,但改良TAP术式X线暴露时间及手术对比剂量使用降低,建议基层医院推广改良TAP技术。Objective To carry out a comparative study on the efficacy of improved TAP technology and DK-CRUSH technology in the treatment of coronary bifurcation lesions.Methods This study included patients with true coronary bifurcation lesions who were admitted to Mianyang Central Hospital from September 2017 to March 2020.They were randomly divided into modified TAP group and DK-CRUSH group,and the main research endpoints between the two groups were compared(re-examination of coronary angiography in-stent restenosis rate at 9 months after operation,major adverse cardiovascular events and stent thrombosis in patients at 12 months after operation,stent thrombosis)and secondary study endpoints(PCI operation success rate,X-ray exposure time,contrast medium dosage,PCI-related myocardial infarction).Results There was no statistical difference of the primary endpoint events,surgical success rate,and PCI-related myocardial infarction between the two groups.However,the X-ray exposure time and the amount of contrast agent in the modified TAP group were significantly lower than those in the DK-CRUSH group.Conclusion The modified TAP technology innovatively combines TAP technology with JBT technology,which is safe and convenient in retaining TAP and reducing costs.At the same time,it reduces the occlusion of the side branches,thereby increasing the success rate of the operation and reducing the incidence of MACE.
