通隧补肾解语汤治疗缺血性脑卒中后失语症临床研究

摘　　要：目的研究通隧补肾解语汤治疗缺血性脑卒中后失语症的临床效果。方法将120例符合标准的缺血性脑卒中后失语症患者随机分为对照组和试验组各60人(实际进入对照组57人,试验组58人)。2组均给予西药及语言康复基础治疗,试验组在此基础上口服通隧补肾解语汤,疗程8周。观察2组治疗前后临床效果、中医证候疗效、美国国立卫生研究院卒中量表(NIHSS)评分和卒中后日常生活能力量表Barthel指数(BI)评分,比较治疗前、治疗后4周和8周的语言功能评分,以及不良反应。结果治疗后,试验组临床疗效、中医证候疗效、BI评分均优于对照组(P<0.05,P<0.01),NIHSS积分低于对照组(P<0.05);试验组治疗后4周和8周的语言功能评分均优于对照组(P<0.05),治疗8周较治疗4周更显优势(P<0.05)。结论通隧补肾解语汤治疗缺血性脑卒中后失语症,具有较好的临床效果,能更好地改善患者失语等神经功能缺损症状,提高日常生活能力。Objective To study the clinical effect of Tongsui Bushen Jieyu decoction in the treatment of aphasia after ischemic stroke.Methods A total of 120 eligible patients with aphasia after ischemic stroke were randomly divided into control group and experimental group,and there are 60 people in each group(in fact,57 people were admitted to the control group and 58 to the experimental group).Each group was given Western medicine and basic language rehabilitation.The experimental group was given Tongsui Bushen Jieyu de-coction on the basis of basic treatment.The course of treatment was 8 weeks.The clinical efficacy and TCM syndrome efficacy,NIHSS score and Barthel index score before and after treatment,as well as the language function score at 4 and 8 weeks before and after treat-ment were observed,and the adverse reactions were also observed.Results After treatment,the NIHSS score reduction and Barthel in-dex improvement of the clinical curative effect of TCM syndrome in the experimental group were both better than the control group(P<0.05,P<0.01),the language function score at 4 weeks and 8 weeks after treatment were both better than the control group(P<0.05),and the advantage was more significant at 8 weeks than 4 weeks after treatment(P<0.05).Conclusion Tongsui Bushen Jieyu decoction has a good clinical effect in the treatment of aphasia after ischemic stroke.It can effectively promote the recovery of neural functions such as language function,better improve the symptoms of neurological function defects such as aphasia,and improve the ability of daily life.
