平喘固本汤合补肺汤对肺源性心脏病患者肺功能、血气指标及炎性因子水平的影响

摘　　要：目的:观察平喘固本汤合补肺汤加减对肺源性心脏病(简称肺心病)患者临床疗效、肺功能、血气指标及血清炎性因子水平的影响。方法:选取肺心病患者112例,按照随机数字表法分为观察组与对照组,每组56例。对照组给予常规西药治疗,观察组则在上述基础上予以平喘固本汤合补肺汤加减治疗,比较两组患者临床疗效,治疗前后肺功能指标[1秒用力呼气容积(forced expiratory volume in first second,FEV_(1))、用力肺活量(forced vital capacity,FVC)、第1秒用力呼气量占用力肺活量比值(forced expiratory volume in first second/forced vital capacity,FEV_(1)/FVC)、呼气峰值流速(peak expiratory flow,PEF)]、血气指标[动脉血氧分压(partial pressure of oxygen in artery,PaO_(2))、二氧化碳分压(arterial carbon dioxide tension,PaCO_(2))]及血清炎性因子[白细胞介素6(interleukin-6,lL-6)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α),超敏C反应蛋白(high-sensitivity C-reactive protein,hsCRP)]的变化情况。结果:治疗后,观察组临床疗效[91.07%(51/56)]显著高于对照组[75.00%(42/56)](P<0.05)。两组治疗后肺功能指标FEV_(1)、FVC、FEV_(1)/FVC、PEF及PaO_(2)水平均升高(P<0.05),PaCO_(2)、血清炎性因子指标lL-6、TNF-α及hsCRP水平均降低(P<0.05),且观察组治疗后上述指标改善情况显著优于对照组(P<0.05)。结论:平喘固本汤合补肺汤加减治疗肺心病患者的临床疗效确切,能显著改善肺心病患者的肺功能和血气指标,并降低血清炎性因子水平。Objective:To observe curative effects of modified Pingchuan Guben decoction and Bufei decoction in the treatment of patients with pulmonary heart disease and its influence on pulmonary function,the levels of blood gas indexes and serum inflammatory factors.Methods:All 112 patients were chosen and divided into the observation group and the control group according to random number table method,with 56 cases in each group.The control group was given conventional treatment of Western medicine,and the observation group modified Pingchuan Guben decoction and Bufei decoction based on the therapy the control group received,to compare clinical effects,the indexes of pulmonary function(FEV_(1),FVC,FEV_(1)/FVC and PEF),blood gas indexes(PaO_(2) and PaCO_(2))and serum inflammatory factors(IL-6,TNF-αand hsCRP)between both groups.Results:After the treatment,clinical effective rate of the observation group[91.07%(51/56)]was higher than that[75.00%(42/56)]of the control group(P<0.05).After the treatment,the levels of pulmonary function including FEV_(1),FVC,FEV_(1)/FVC,PEF and PaO_(2) were elevated in both groups(P<0.05),the levels of PaCO_(2),serum inflammatory factors such as IL-6,TNF-αand hsCPR were lowered(P<0.05),and the indexes above of the observation group were significantly better than these of the control group(P<0.05).Conclusion:Modified Pingchuan Guben decoction and Bufei decoction could gain definite clinical effects in the treatment of patients with pulmonary heart disease,which could improve pulmonary function and the levels of blood gas indexes,meanwhile reduce the levels of serum inflammatory factors.
