膝关节后交叉韧带胫骨止点撕脱骨折重建术后中医特色康复及护理研究

摘　　要：目的:观察中医特色康复护理在膝关节后交叉韧带胫骨止点撕脱骨折重建术后的效果。方法:选择90例膝关节后交叉韧带胫骨止点撕脱骨折重建术后患者,根据不同护理方法分为对照组和观察组,每组45例。对照组给予常规康复护理干涉,观察组在对照组的基础上给予中医特色康复护理干涉,共干预4周。观察两组负性情绪、疼痛评分、睡眠质量、安全性及膝关节功能、膝关节活动度、护理满意度。结果:负性情绪评分两组护理前比较,差异无统计学意义(P>0.05);护理后,两组均降低(P<0.05),且观察组较对照组降低更明显(P<0.05)。术后3、7天两组疼痛评分、睡眠质量评分均较术日及术后1天明显改善,且观察组较对照组明显改善(P<0.05)。并发症发生率观察组[11.11%(5/45)]明显低于对照组[28.89%(13/45)](P<0.05)。术后4周两组膝关节功能和膝关节活动度较术前明显改善,且观察组较对照组明显改善(P<0.05)。护理总满意度观察组[95.56%(43/45)]明显高于对照组[80.00%(36/45)](P<0.05)。结论:中医特色康复护理可有效调节膝关节后交叉韧带胫骨止点撕脱骨折重建术后患者的负性心理,改善疼痛程度和睡眠质量,提高膝关节功能和膝关节活动度,增加患者满意度。Objective:To survey the effects of TCM-featured rehabilitation nursing after reconstruction of posterior cruciate ligament tibial avulsion fracture of knee joint.Methods:Altogether 90 patients were allocated to the control group and the observation group with 45 cases in each group according to different nursing methods.The control group adopted conventional rehabilitation nursing,and the observation group TCM-featured rehabilitation nursing on the foundation of conventional rehabilitation nursing,for four weeks of intervention,to observe negative emotions,VAS,quality of sleep,the safety,knee joint function,knee joint mobility and nursing satisfaction degrees between both groups.Results:The difference had no statistical meaning in the scores of negative emotions between both groups before the nursing(P>0.05);the scores reduced in both groups after nursing(P<0.05),the decrease of the observation group was more significant than that of the control group(P<0.05).VAS scores and the scales of quality of sleep three and seven days after the surgery improved more evidently than the operation day and one day after the surgery,and the observation group improved more obviously than the control group(P<0.05).The incidence of the complications of the observation group was[11.11%(5/45)],remarkably lower than[28.89%(13/45)]of the control group(P<0.05).Four weeks after the surgery,the improvements of knee joint function and knee joint mobility of both groups were more remarkable than before the operation,and the improvements of the observation group were more significant than these of the control group(P<0.05).Nursing satisfaction degree of the observation group was[95.56%(43/45)],evidently higher than[80.00%(36/45)]of the control group(P<0.05).Conclusion:TCM-featured rehabilitation nursing could effectively adjust negative emotions in patients after reconstruction of posterior cruciate ligament tibial avulsion fracture of knee joint,improve VAS and quality of sleep,raise knee joint function and knee joint mobility and increase
