化瘀复膜方在宫腔粘连术后患者中的应用效果及对雌孕激素受体及血流参数的影响

摘　　要：目的:探讨化瘀复膜方对宫腔粘连术后患者雌孕激素受体及血流参数的影响。方法:选取2018年1月-2020年8月于河南省中医院治疗的84例宫腔粘连术后患者,按随机数字表法分为对照组和观察组,每组42例。两组均行宫腔镜下粘连电切术,术后对照组口服戊酸雌二醇片、黄体酮胶囊,观察组在对照组基础上联合化瘀复膜方(灌肠)。观察两组总有效率与月经恢复率、雌孕激素受体表达情况、子宫内膜厚度与动脉血流。结果:观察组总有效率、月经恢复率高于对照组,雌激素受体、孕激素受体、细胞增殖蛋白Ki67水平均高于对照组(P<0.05),子宫内膜厚度大于对照组,动脉血流收缩末期峰值与舒张末期峰值比值、搏动指数、阻力指数均低于对照组(P<0.05)。结论:化瘀复膜方灌肠可改善宫腔粘连术后患者子宫血流情况并促进子宫内膜增厚,有助于促进月经恢复和雌孕激素受体的表达,提高治疗效果。Objective:To investigate the influences of the Huayu Fumo decoction(化瘀复膜方)on estrogen and progesterone receptors and blood flow parameters in intrauterine adhesions patients after surgery.Methods:Eighty-four cases from January 2018 to August 2020 were selected and divided into the control group and the observation group according to the random number table method.Both groups were given hysteroscopic resection of adhesions.After operation,the control group were given estradiol valerate tablets plus progesterone capsules orally,while the observation group were additionally given enema with the Huayu Fumo prescription.Results:The total efficiency and menstrual recovery rate in the observation group were higher than those in the control group.The levels of estrogen receptors,progesterone receptors,and cell proliferation protein Ki67 in the observation group were higher than the control group(P<0.05).The endometrial thickness in the observation group was greater than the control group.The ratio of end-systolic peak arterial blood flow to end-diastolic peak arterial blood flow,pulsatility index,and resistance index in the observation group were lower than the control group(P<0.05).Conclusion:Enema with the Huayu Fumo prescription can ameliorate uterine blood flow in patients after IUA,promote endometrial thickening and menstrual recovery,and improve the expressions of estrogen and progesterone receptors and the therapeutic effect.
