十珍汤在眼科疾病的应用

摘　　要：十珍汤为眼科古方,出自《审视瑶函》。《审视瑶函》首刊于明崇祯十七年(1644年),是中医眼科古籍文献中的代表之作,由明代医学家傅仁宇撰写。十珍汤由生地黄、当归、知母等药组成,适用于虚损血枯,肝肾亏虚证,专治赤痛目病。经临床实践验证,文章认为十珍汤非常适合治疗肝肾亏虚型眼病,认为有眼睛赤痛的表现,可能与肝肾相关,肝肾亏虚,阴虚火旺,虚火上炎,则目赤痛,复感外邪,临床容易被表现在外的症状所迷惑,误辨为表实证而予以炎凉药,虽可治标,却忽视内虚,反而中伤脏腑,病越严重。基于“异病同治”原则,临证只要辨证准确,确定核心病机,采用此方往往会取得立竿见影的效果。文章结合案例,分享十珍汤治疗眼科疾病经验,以期为临床拓展思路。The Shizhen decoction(十珍汤),an ancient prescription of ophthalmology,comes from Shenshi Yaohan(《审视瑶函》).Shenshi Yaohan was first published in the 17th year of Chongzhen(1644)Ming Dynasty.It is a representative work in the ancient literature of ophthalmology of TCM,written by FU Renyu,a famous doctor in Ming Dynasty.The Shizhen decoction is composed of Dihuang(Radix Rehmanniae),Danggui(Radix Angelicae Sinensis),Zhimu(Rhizoma Anemarrhenae)and other drugs.It is suitable for deficiency of blood,liver and kidney deficiency syndrome,and specializes in treating eye disease.Verified by clinical practice,this paper argues that the Shizhen decoction is very suitable for the treatment of liver and kidney deficiency type eye disease.It is believed that the performance of red pain in the eyes may be related to the liver and kidney,such as liver and kidney deficiency,Yin(阴)deficiency and fire exuberant,weak fire inflammation,resulting in eye red pain.Because of complex sense of external evil,it is easy to be confused by the symptoms of the outside,and the disease is mistakenly identified as the demonstration of exterior excess syndrome and is treated with Yanliang medicine(炎凉药).Although it can treat the symptoms,ignore the internal deficiency.In this way,the viscera will be hurt,and the disease will be aggravated.Based on the principle of homotherapy for heteropathy,as long as the syndrome differentiation is accurate and the core pathogenesis is determined,the use of this prescription will often achieve immediate results.In this paper,the experience of Shizhen decoction in treating ophthalmic diseasesis shared to expand ideas for clinical practice.
