苍附导痰汤加减辅助治疗肥胖型多囊卵巢综合征临床疗效系统评价

摘　　要：目的:系统评价苍附导痰汤加减辅助治疗肥胖型多囊卵巢综合征(Polycystic Ovary Syndrome,PCOS)的临床疗效。方法:应用计算机检索相关中英文数据库关于苍附导痰汤加减辅助治疗肥胖型PCOS的随机对照试验(Randomized Controlled Trials,RCT),各数据库检索时间从建库初至2022年5月。对入选文献进行质量评价和数据统计,使用RevMan 5.4软件进行Meta分析。结果:共纳入文献11篇,患者1018例。Meta分析结果显示:中西医治疗组的临床总有效率[OR=4.47,95%CI(2.97,6.72),P<0.00001]、身体质量指数(Body Mass Index,BMI)水平[MD=-3.72,95%CI(-6.41,-1.03),P=0.007]、排卵率[OR=2.62,95%CI(1.76,3.89),P<0.00001]、睾酮值[SMD=-0.72,95%CI(-0.87,-0.58),P<0.00001]、促黄体生成素值[SMD=-1.15,95%CI(-1.35,-0.95),P<0.00001]等性激素水平均较对照组有明显改善,且不良反应较少。结论:现有证据显示苍附导痰汤加减辅助治疗肥胖型PCOS疗效显著,相对单纯西药治疗能明显提高总有效率及排卵率,改善BMI以及性激素水平,且无明显不良反应,可为进一步临床推广提供依据。Objective:To systematically evaluate the efficacy and safety of the Cangfu Daotan decoction(苍附导痰汤)in the adjuvant treatment of obese polycystic ovary syndrome(PCOS).Methods:Randomized controlled trials(RCT)of the Cangfu Daotan decoction for the adjuvant treatment of obese PCOS were searched in relevant Chinese and English databases by computer.The retrieval time was from the beginning of each database to May 2022.Quality evaluation and data statistics were conducted for selected literatures,and meta-analysis was conducted using RevMan 5.4 software.Results:The research showed that the effective rate in the combined group was higher significantly[OR=4.47,95%CI(2.97,6.72),P<0.00001],and the BMI level was lower compared with the Western medicine group[MD=-3.72,95%CI(-6.41,-1.03),P=0.007],ovulation rate in the combination group was higher[OR=2.62,95%CI(1.76,3.89),P<0.00001].T values[SMD=-0.72,95%CI(-0.87,-0.58),P<0.00001],LH values[SMD=-1.15,95%CI(-1.35,-0.95),P<0.00001]were better than the control group,with fewer adverse reactions.Conclusion:The available evidence shows that the Cangfu Daotan decoction is effective on obese PCOS,significantly increasing the overall efficiency and ovulation rate,improving BMI levels and sex hormone levels,without significant adverse effects.
